The worker was striking the head of a bolt with a hammer to unplug a 1 ¼ X 4" pipe nipple. When the material inside the pipe nipple broke loose the impact of the bolt pinched the thumb of the left hand against the pipe nipple. The worker was transported to the local emergency room and received 5 stitches to the thumb.

Employee has seen several specialists throughout months, MRI, X-ray to determine source of pain/issue. 3/18/20: doctor finally placed a hard cast on left wrist and gave cortisone shot.

FLW was removing an idler pulley from a piece of tracked equipment. While striking the pulley with a 3# hammer a shard of metal came off of the hammer and struck the worker in upper thigh.

Employee was repairing a drive belt, and attempting to loosen a nut, using a crescent wrench. While employee was applying pressure to loosen the nut, employee felt a pain in right shoulder.

Party injured left shoulder while trying to lift the lid on one of the sand-trucks. Lid was bound close and the 3ft' pry bar-like tool was used. Employee stated EE may have jerked back EE's arm to hard and felt EE's shoulder pop. Later that night it started causing EE pain.

Working on bend pulley on overlay conveyor, load shifted and caught maintenance workers index finger. EE was able to pull finger out of the glove, but the tip of the finger was bleeding and first aid did not stop it. Doctor visit confirmed tip of finger was fractured, but did not stitch or glue the finger. A splint was placed, no work restrictions.

Mechanic was cleaning out a dust collector hopper on 6/6/19. EE was using a deadblow hammer to hit the discharge cone. EE used the hammer for about 30 minutes. Later that day they felt tightness in the right elbow, and reported to supervisor. Employee attempted to treat at home using ice/heat and rest, but aggravated injury bagging paper bags on 6/19/19.

Employee was removing a guard on the tail pulley of a belt conveyor. The last bolt on the guard became wedged so the employee tried to free the bolt using a maul and another bolt as a punch. When EE swung the maul to free the bolt, EE missed and struck finger resulting in a fracture.

IE and four other EE's were working to break the ice in the flex pipe lines on the hydrosizers. They had all became frozen solid, and we were removing the ends by undoing the clamping on the end, and knocking the end off of the fitting using a dead blow sledge hammer.

Employee experienced severe back and chest pain while shoveling sand spilling from a dryer belt. The employee collapsed onsite, EMS transported them to the Kermit Memorial Hospital. Employee was evaluated and released to return to work on Jan. 14th. The employee sought additional medical evaluation; subsequently treated for muscle spasms with followup scheduled for 1/23/19

While removing a 2" diameter pipe using a 18" pipe wrench, a 2" connector broke. The pipe which was under pressure broke loose and struck EE on EE's left eye brow causing a laceration. The employee was examined, treated, and eight sutures placed above EE's left eye.

While cutting a piece of rubber tubing, employee, using a utility knife, sliced the rubber towards EE's body instead of away, the blank ran up EE's thumb opening up a one-inch cut. EE was taken immediately to the local hospital and was given aid. The attending physician sewed up the cut with 8 stitches and immediately released EE to full duty.

Employee was using a 24" crescent wrench to loosen a bolt securing the gearbox that was needing to be changed. The wrench slipped off the nut causing EE to lose balance and fall on to EE's rear end. Employee chose not to receive medical attention at this time.

Blasters were loading a shot in a heading, a cord needed to be cut so the EE asked another blaster to cut the cord while EE held it. The EE moved slightly when the cut was about to be made and cut finger. EE was taken to a clinic and received stitches.

EE was attempting to loosen a nut with a wrench. Nut was diffucut to break loose EE felt pain in the back. EE was taken to ER X-rays were taken. ER reported Muscle strain of the upper and mid back with possibility of rib factures.

Employee was using a 3 LB. hammer to drive a pin into the wear plate on the bucket of the 988G. The hammer hit the side of the top of the pin and broke a metal piece free. The metal chip flew into the left forearm of the employee. 2 stitches were required for the cut.

After removing the retaining screw, the lock ring on the fines screw was still stuck. The employee used a chipping hammer and a 4lb sledge hammer to tap it loose. When the employee struck the chipping hammer with the sledge hammer, a piece of metal broke off the tip of the chipping hammer, went through his leather glove and into his left thumb.

Employee was prying on a rod mill liner with a pry bar and holding it up. The liner shifted, releasing the pressure on the bar. The liner shifted, fell and caught the bar. The bar flew up and hit employee in left cheek.

While working on a pump in the maintenance shop, the EE had his hand placed on top of the pump, lining up the chuck in the shaft. Another employee had a pry bar lifting on the opening of the shaft, when the bar slipped, striking the injured employee's hand, breaking the metacarpal bone in his right hand.

The employee was attempting to remove a 3/4" bolt with a socket wrench and the bolt broke causing his hand to strike against a piece of angle iron that was adjacent to the bolt. This caused a laceration that required four stitches. X rays were negative and the employee returned to work with no job restrictions or modifications.

In the process of removing bucket tips on a 385. Retainers were removed and tips were stuck to adapters. While using 10 lb sledge hammer to loosen a sliver of metal split off and struck employee in left leg above knee.

A wet plant Utility was assisting EE with replacing v-belts on VS-01. The utility was using a long bar to hold the motor up to release tension on the belts to allow replacement. EE was using his bare fingers to "walk" the new belts onto the pulley. At some point the bar slipped and pinched EE's left index finger between the belt and the pulley causing a laceration.

EE was opening a rail car door with a metal bar and his hand slipped causing his middle knuckle on his right hand to strike the car. At the time of the incident he felt no pain. On 10-3-11 he was taken to the hospital and the X-ray was negative and was considered first aid. He had a medical procedure on 10-25-11 that made this reportable. No lost time, No modified duty.

Employee was removing a bearing race that was "galled" to the shaft. The metal had been burned/cut with a torch in two places. The helper proceeded to strike the race with a 4-lb hammer to release it from the shaft. A small piece of metal flew off and struck employee in the left thigh which resulted in a puncture wound and the metal being lodged in his leg.

Two employees were working together taking concrete forms down and cleaning and stacking them. One employee was using a hammer to remove the concrete from the form and the injured employee thought he was done, grabbed the form and was hit on the pinky finger. The employee did not see his hand there until it was too late. It broke his pinky finger.
