EE was changing out a caulk tube in the caulk gun. EE went to puncture tube with the metal puncture rod (which was unusually sharpened) slipped and poked their left hand (between thumb and index finger).

Showing all 20While working with a hammer on metal machinery parts, an airborne metal splinter lacerated the employee's cheek. First aid measures were applied but after a few days it was determined that a piece of the shard remained in the wound. On 1/20/21 an outpatient procedure was performed to extract the shard, and the employee was released to work without restrictions.
Employee was standing on the ground using a hand tool to clean mud from the exterior of a trailer. Following this activity, the employee experienced pain and limited range of motion in their shoulder. After first aid measures did not resolve the issue, a physician recommended restricted duty accommodations pending further diagnosis.
On 03/18, EE reported that EE was having back pain that potentially stemmed from a task EE performed on 03/11. On the evening of 03/11, the employee was using a metal rod to clear debris from a bin level sensor. Employee states pain was minor starting on 03/12 but worsened to the point of reporting on 03/18. Incident reclassified to Recordable on 03/20 after visiting clinic.
An employee was barring out a chunk of frozen material caught between the conveyor belt and the tail end roller. The employees left hand got pinned between the bar and the bracing of the conveyor, lacerating the palm. The employee was taken to the hospital where EE received stitches and a tetanus shot. The employee was released to return to work under normal duties.
Employee was having a hard time closing the bottom door on a railcar. They grabbed a hammer to hit the door, the hammer handle ricocheted and hit the EE in the mouth chipping front right tooth.
Small metal shard from hammer flaked off and entered hand. Was removed by doctor and 4 stitches to close
was positioning final drive, swung over and smashed finger between ripper cylinder and final drive on dozer. Finger is cut (no stitches) and bruised
Third party temporary lab technician working un-supervised, took her personal pocket knife out to punch a new hole in her belt and cut her left index finger. She was given 6 stitches and prescription pain medication to take as needed at home off duty; and was released for full work duty.
As EE was lowering pickup, jack handle slipped out of their hand hitting EE in the safety glasses, cutting the left side of their nose. On 01/18/2010 this moved from a First Aid to a Reportable due to EE seeing a Specialist due to Cartilage Damage to the nose.
The employee used a rounded tip box blade cutting tool to cut a bad section of a hose out. Dust blew from the hose and into the face of the employee causing the employee to cut his hand with the cutting tool.
EE was banding a boxcar with another co-worker, using utility knife to cut fiber bands. Injured stepped back into path of knife arc swing of co-worker.
Attempting to put bottom gate of railcar onto track.Using a steel bar, pound gate back onto track.Holding steel bar as a rammer to knock the gate into track.Steel bar slipped off gate causing finger to be pinched between the bar & railcar causing a laceration to left index finger.Received 6 sutures, given prescription for antibiotics & painkiller, restricted duty 10 days
EMPLOYEES WERE CHANGING JOURNAL ASSY. UPON INSTALLING REPLACEMENT JOURNAL, THE TOOL USED TO REMOVE / REPLACE JOURNALS SLIPPED OFF JOURNAL & POPPED UPWARD SMASHING EMPLOYEES FINGER BETWEEN TOOL & BOLT.
EE WAS DRAGGING ASH CLEANING #1 FURNACE. HE SMASHED HIS RIGHT INDEX FINGER BETWEEN THE PULL RAKE AND THE ASH BIN WHEN HE TOSSED RAKE INTO THE FURNACE TO PULL OUT MORE ASH. EE WAS TAKEN TO ER A T HOSPITAL WHERE HE RECEIVED TREATMENT INCLUDING STITCHES. EE RETURNED TO WORK AND FINISHED SHIFT. EE DID NOT COME INTO WORK THE FOLLOWING DAY DUE TO PAIN.
INJURED WAS ATTEMPTING TO CLOSE LEAKING BOTTOM GATE ON BULK C-6 RAILCAR USING A SLEDGEHAMMER. HE PLACED HIS LEFT FOOT AGAINST GATE TO GIVE IT CONTANT PRESSURE. WHILE TRYING TO GET GATE CLOSED, HAMMER HEAD GLANCED OFF CAR STRIPKING TOP OF HIS LEFT FOOT, JUST ABOVE HIS STEEL TOE CAP.
EE WAS WORKING ON THE COMBINE FEEDER REPAIRING ABROKEN CHAIN. HE WAS USING A BAR TO PULL THE IDLER PULLEY OVER AND THE BAR SLIPPED OFF CAUSING THE EE TO HIT HIS FINGER BETWEEN THE BAR AND THE FEEDER CAUSING A LACERATION TO HIS FINGER.
EE WAS CUTTING A STRING OUT OF A BAG WITH A BOX KNIFE. HE WAS CUTTING ABOVE HIS HEAD AND WHEN HE CAME DOWN WITH THE KNIFE IT CAUGHT HIS FINGER, RESULTING IN A LACERATION THAT REQUIRED STITCHES
EE ALLEGED WHILE LINING UP BOLT HOLE IN THE MILL WALL. HE STRUCK A SPUD BAR WITH A HAMMER & A SMALL CHIP FLEW OUT & LODGED IN HIS LOWER CHEST AREA.
