Over-exertion in pulling or pushing objects

On 03/18, EE reported that EE was having back pain that potentially stemmed from a task EE performed on 03/11. On the evening of 03/11, the employee was using a metal rod to clear debris from a bin level sensor. Employee states pain was minor starting on 03/12 but worsened to the point of reporting on 03/18. Incident reclassified to Recordable on 03/20 after visiting clinic.

Struck against stationary object

Employee lacerated the middle finger on right hand while installing a brash bushing into a journal. Employee was taken to the hospital and was treated and released without restrictions. The physician prescribed antibiotic medication.

Over-exertion in lifting objects

Employee was lifting 20lb lid to bucket elevator and felt discomfort in his lower back. Employee was seen by a physician who determined it to be a strain. Employee was given prescription medication and released back to full duty. The employee indicated he had been experiencing intermittent flares of lower back pain, including an episode two months ago after a baseball game.

Third party temporary lab technician working un-supervised, took her personal pocket knife out to punch a new hole in her belt and cut her left index finger. She was given 6 stitches and prescription pain medication to take as needed at home off duty; and was released for full work duty.

Over-exertion in lifting objects

Third party worker was transferring bags of barite material to a new pallet because there was a busted bag at the bottom of a stacked pallet. Worker felt discomfort in his right shoulder blade. Worker was sent to medical by his agencies company services.

Fall to the walkway or working surface

Employee stepped on a partially covered inspection door and fell back hurting his left knee. Employee incident occurred on 05/31/2012. First aid measures were administered. After further medical evaluation, treating physician put him on restricted duties on 06/28/2012.
