Employee was closing hatch lid on control valves of Prill truck when it slipped out of his hand and dropped on left forearm which was underneath it laying down the support bar.
Small metal shard from hammer flaked off and entered hand. Was removed by doctor and 4 stitches to close
Dome light bulb in CAT 777B haul truck popped and got some glass in employee's eye. Glass was washed out but apparently resulted in chemical conjunctivitis for two days of restricted duty
Employee jarred his back in a pothole while driving a haul truck. Employee claims inability to continue to work. Under medical review and care.
While performing normal duties as a driller, the employee felt a gradual strain develop in his back
Driller failed to remove deck bushing from upper sub per procedure and while trying to thread the 2nd section of steel the top of the carousel hit the safety wrench, breaking the chain which allowed the wrench to fall, glancing off the helper's hardhat the as he was visually checking to ensure that the steel was threading properly positioned outside of the cab on the drill deck.
Employee was welding wear iron onto the bottom of a detached loader bucket. He got help to lift steel onto tabs already in place to support steel, when one failed allowing it to fall to the ground catching his glove on the way down trapping his arm underneath. Resulted in a significant bruise to left forearm requiring anti-inflammatory prescription and follow-up physical therapy.

Employee was winching up the lid of a dumpster with a boat winch. When the employee let go of the winch handle it did not lock and spun back striking the employee on his left ring finger causing a laceration. Finger needed sutures.
Operator of a haulage truck lost control while descending down ramp. Operator drove truck through an engineered berm in lieu of emergency braking. As a result, the truck tipped on its side and the operator banged his head resulting in eight staples.

Climbing up ladder to clean mirror, foot slipped on rung and scraped leg on the edge of equipment.
Employee went to doctor after he was terminated for cause and claimed he hurt his shoulder while shoveling and lifting at work. (Questionable claim).
Injury arose while employee fell into a jig while holding 25lbs. of steel in his arms. The injury paperwork was not received until 4/17/12.
During the pre-shift inspection of his haul truck the employee slipped from the same level and fell on his back/tailbone.

While climbing up to the cab of a track hoe the employee felt a sharp pain in his left calf. He said he didn't slip or anything else; he just felt a pull in the muscle.
The operator was backing up a 777 haul truck and being loaded by a 242 loader. It is not known if the top of the bucket hit the top of the bed of the truck or if the load caused a force resulting in the operator banging their head on the drivers side window. At the time of this report, a second objective medical finding had not been obtained.
Employee was removing a bolt with an air impact wrench and holding onto the nut with his other hand. The nut spun in his gloved hand and a sharp edge on the nut cut through the glove lacerating his 3rd, left finger. The employee received three sutures.
While changing cutting edges on a WA500 loader bucket,Mechanic employee was installing bolts with hand underneath bucket. Loader was idling operator was sitting in cab. Operator dropped his hand held radio as he reached for it, his coat caught on the steering wheel causing the loader to turn and drop bucket onto mechanic employee hand.

Taking down a gear box from a conveyor belt. The employee states that the gear box ""slid"" and made contact with his finger causing the fracture.
The auger on the ANFO truck was stuck. EE removed the inspection plate cover from the vertical auger and put his hand inside to remove the blockage. The auger moved pinching his fingers between the auger and the auger housing.

While steam cleaning a radiator on a D-8 dozer the employee slipped on the wet track and fell onto the stailizer bar. He thought he just bruised his ribs and continued to work until seeking treatment on 07/13/2009. An X-ray revealed a small fracture to one of his ribs.

While changing teeth on loader, pin punch broke & fragment struck EE on chin. Cut needed 1 stitch no impairment.
Moving drill over hole to redrill, stopped and stood on deck to check alignment. Collar was stuck at top of pipe. Collar came down and cut thumb.
Backed truck under shovel to be loaded. Shovel swung around and hit rear side of truck.
Going up cut above shovel. Dozer peaked and front end came down jarring operator.
Coming out of bathroom, slipped on ice & fell, sprained ankle.

Contract employee was stepping down off of pickup truck to ground. Improper foot placement resulted in contract employee twisting right knee, injurying it.
Crusher operator was trying to redirect a very large rock into the jaw crusher, when it fell the wrong way and contacted his left toe. The rock impacted his steel toe boot into his left big toe
Employee was unplugging cone crusher when he felt a pain in his right hand between his thumb and first finger. Cold pak was applied on site, employee finished working his shift. After employee had returned home swelling returned and he went to the emergency room for evaluation. Was placed on light duty.

EE STATED THAT HE WENT INTO A STORAGE CONTAINER TO GET A PENTIL CHAIN OFF OF A SHELF FOR A PAN FEEDER REPAIR IN PROGRESS. WHEN HE LIFTED THE CHAIN HE FELT A SLIGHT PAIN IN HIS BACK. AFTER HIS SHIFT ON THE WAY HOME THE PAIN GOT WORSE. THAT EVENING HE WAS TAKEN TO THE HOSPITAL.

A ROCK THAT HAD BEEN LIFTED UP OUT OF THE JAW CRUSHER ONTO A DECK ROLLED BACK ONTO EE'S FOOT.
