Pulling Poly Deck panels out using pin extractor tool. While hammering tool into place the tool popped out from poly deck panels. Struck employee in the mouth causing their lip to bleed and chipping a tooth.

EE was working on the Dewatering Wheel when their wrench slipped and struck them in the chin. EE sustained a small laceration and was taken to the ER to be evaluated where they received 3 stitches.

A miner was cutting a piece of skirt board rubber when the belt knife slipped causing them to cut through their jeans into their thigh. The cut was not bleeding but clinic placed two stiches on the cut to prevent it from opening up.

A miner was working underneath the Jaw Crusher swinging a Sledge Hammer to knock out a piece of metal on May 27th when EE strained EE's lower back. The miner had previously reported EE's back was bothering EE on 5/11/2022 while working at the mine. At that time no restrictions were put in place and it was not reportable. The miner had previous unrelated back injuries.

While installing a new feeder, there was a rigging failure on a come along which resulted in the cables failing and dropping the feeder on EE's toes. EE was wearing steel toed boots, which prevented serious injury, resulting in EE's big toes being broken. EE is on limited light duty as EE is temporarily in a walking boot.

EE was getting a guard off some bolts, and the pry bar slipped and smashed EE's right ring finger. Tip of finger had a small fracture and the finger nail had to have a hole poked into it to let blood drain out from under finger nail.

Using a Come Along to pull a piece of metal into place to weld it. EE was jacking the Come Along & a link in the chain on the Come Along failed, springing back and struck EE in the face-above eye and on forehead.

Rocks collected in a chute going to screen deck. EE was removing rock from the chute, using a spud bar. The bar slipped, pinching a finger between the bar and a beam supporting the screen deck. EE sustained a angulated & rotated fracture to the left 5th (pinky) finger, requiring a splint and possibly surgery.

Changing hydraulic hose, wearing insulated leather gloves. EE braced a crescent wrench against the handrail on the manlift basket. While applying pressure to the pipe wrench on the coupling, the coupling broke loose & EE's LT hand struck the rail of the manlift, cutting pinky fingertip DIP joint on palm side of left hand.

Maintenance work-pin removal. While driving pin from a hydraulic ram, a metal chip came off when the hammer struck the punch, penetrating right thigh, requiring sutures.

Using a pry bar to get a cable unstuck on a loading spout for the south mineral filler bin. The cable slipped, causing pry bar to come off spout, his RT hand came in contact with an angle iron, across the top of 5th finger & knuckle. Fracture to right 5th finger. Splinting. Light duty restrictions.

Using knife to cut a notch in belt to replace a belt scraper. He drew knife towards his body, knife slipped & cut him in upper right leg. Treated at Occ Health, sutures received and removed. No work missed.

Employee was tightening a large bolt with a ratchet. The ratchet slipped and employee's right hand struck solid steel structure smashing his right pinky finger between the structure and the ratchet. Employee's right (pinky) 5th finger was crushed. Employee was restricted.

Employee was cutting rubber skirt for an 8 X 20 wash screen. Employee was cutting a hole for a spray pipe to go through the skirt. Employee was finishing the inside corner when the knife cut out and contacted employee's left thumb on the first knuckle resulting in a laceration. Three stitches were required.

Employees were getting ready to splice a belt and injured employee was going to move the tail pulley up to give the belt a little slack. As employee was adjusting the tail pulley, the wrench slipped off and came back and struck employee in the mouth/teeth. Employee received a chipped tooth and had to seek dental treatment.

Employee was using a wrench to take a bolt loose at the wash deck. The nut broke loose and employee's wrist hit a bolt down below causing a sprain and contusion to his left wrist. Employee is restricted. No lost time.

Employee was repairing the screen deck and was working on a piece of steel to bend it backwards using a sledge hammer. The sledge hammer bounced and employee lost grip and his right hand slipped and struck a metal burr on sheet of steel. Employee received a laceration to his right 5th finger. Stitches were required. No lost or restricted time.

Employee was changing the brake pads on the NW 95 dragline hoist brakes. A pry bar fell over in to the drum and came around and hit employee's arm. Employee received a severe fracture to his right arm and abrasions to his left hand. Had surgery and is off work.

Employee was using a sledge hammer and pry bar to remove a pin from the tooth of the Hitachi shovel when the sledge hammer slipped off the pry bar and hit employee's left index finger. Employee received a crush injury to his finger and later contacted cellulitis. Employee is restricted. No lost time.

Employee was cutting a belt from a catch-all with utility knife. Employee was pulling down on the knife when the knife ripped through the belt and continued on in motion and struck his right leg. He received a laceration/puncture to his right leg and received staples/stitches.

Employee was using a punch and hammer to punch holes in side of the wipers on the conveyor belt. Employee missed the punch and hit his left thumb with the hammer. Employee went home but the next morning he could not move his thumb and was sent to the company clinic. He received a contusion to his left thumb.

Employee was using a chipping hammer to knock slag off the weld. A piece of hot slag flew up and went under employee's safety glasses and hit his left eyeball. Employee received a foreign body in the Cornea. Employee was restricted at first but later had to be off work.

Employee was taking the bucket teeth off of the dragline bucket. He was using a hammer to dislodge the bucket tooth when a piece of steel from the tooth chipped off and went into employee's right thigh. The piece of steel was embedded in his leg and he has to have surgery to have the piece of steel removed.

Employee was working on gates at the front entrance. He was using a hammer to drive a pin out of a hinge on the front gate. A piece of metal flew up and hit employee on the chin. The piece of metal went through his chin and chipped a lower tooth. Dermabond was used to close the laceration in his chin.

Employee was under the fuel tank using a wrench to tighten a bolt on the vertical swing shaft on the dragline. Employee felt pain but did not report it until two days later. He thought he would be o.k. but had to go to the doctor. He recieved a right shoulder rotator cuff-Tendonitis. Employee is restricted.

EE WAS TAKING LUGNUT OFF HAULUP TRUCK. EE WAS USING A WRENCH. THE EXTENSION FROM THE BREAKER BAR TO SOCKET BROKE AND A PIECE OF THE WRENCH FLEW UP AND HIT HIS MOUTH BREAKING 10 TEETH. FRACTURE TO LEFT JAW BONE AND CUTTING THE INSIDE OUTSIDEOF HIS MOUTH. HAD 50 STITCHES INSIDE OF MOUTH AND 8 ON OUTSIDE.
