While removing concrete from a core drilling operation, the tool used to remove the concrete core slipped and pinched the employee's hand. EE's right hand was caught between a piece of plywood and the rebar causing a laceration to EE's right hand through EE's glove.

Showing all 45A new production control supervisor (PCS) was assigned the task of changing valve positions in the buffer silos. The PCS used the incorrect tool for the task (too large). When tapping the actuator ee struck/pinched the tip of right ring finger between the tool and a pipe flange. The injury is a laceration and fracture of the finger tip.
The EE was working with a vacuum truck crew cleaning spillage. EE used a utility knife to cut through duct tape at the joint of one hose. The blade became stuck and EE exerted upward force. The knife came free of the tape and struck EE in the rt leg just above the knee. The injury is a laceration. First aid was applied.
Electrician was stripping cable and using leg as a brace while stripping. Knife slipped off cable and cut leg through the pants making a 3" incision requiring stitches.
Employee was attempting to cut a zip tie from a flange when knife slipped off and stabbed self in the left wrist. Employee was taken to Albany Med urgent care and received 4 stiches. Return to work after the incident with no restrictions.
EE was trimming packing material with a serrated knife for a tail bearing installation on FM3 dust collector. EE stated was not having efficient success, so switched to using a box cutter. During the process of cutting action (towards body in a sitting position) the box cutter slipped off material, then cut a three inch gash in upper thigh.
EE rigging comealongs when repairing drag conveyor. The comealongs were anchored to a steel pipe whick broke striking the EE in the head. EE was stabilized and transported by ambulance to local ER. He did have a concussion. EE was examined and released later in the evening.
A production employee twisted or sprained his back while clearing buildup in the kiln system.
A millwright injured his nose when a handle on the manhead broke off and struck him on the bridge of his nose. The weld holding the handle failed causing it to break.
EE was attempting to clear a jam on Raw Mill No. 2 circ pump with modified 4' pipe wrench and come-along on shaft of locked out pump when the pump housing shattered. The EE was struck on the left shoulder by the pipewrench.
An employee strained his back while loosening a high pressure water line connection.

A Production Attendant was tightening a bolt on the petcoke coriolis when the wrench he was using slipped off a struck him in the forehead.
Employee was loosening a fitting on a 2" pipe and noticed a pain in his lower back. Employee contacted medical provider by phone and was given home care instructions. The employee called again on 7/5/08 and still had tightness in his lower back. He was seen by a physician on 7/7/08 and prescribed medication for the muscle strain.
Working on housekeeping in the east zeno pit with a shovel when a object on the floor was struck and he jammed his wrist.
EE was trying to unjam the East basin pumps by pushing on a 4' pipe wrench when the wrench slipped on the shaft and his hands slipped on the wrench (gloves were wet) and he lost his balance.

As technicim was making a downward blow w/hammer. the head struck the suppressiom system on the d-11 caught the handle to come toward the technicion. The end of the handle struck the left inner thigh area between the groin and knee cap, resulting in severe contusion.
Injured EE was tightening bolts on upper section of vertical roller. He was working on a ladder in an awkward position holding the nut with his lt. hand and working a ratchet with his rt. hand. EE leaned over and around conveyor bedframe and roller mount. He felt a ripping sensation in his left shoulder.
EE cut middle finger of rt. hand with utility knife, when cutting section of hose from vacuum truck tak-up port.

MAINTENANCE MECHANIC WAS USING A POWER PACK HYDRAULIC JACK TO REMOVE THE CAP FROM A BEARING. A NUT THAT HE WAS USING AS A SPACER BROKE UNDER PRESSURE & HIT HIM IN THE MOUTH. HIS BOTTOM LIP WAS CUT & REQUIRED 2 SUTURES.
OVEREXERTION WHILE USING MANUAL CRIMPER ON CABLE. RESULT IS UMBILICAL HERNIA.
SUPERVISOR TRIED TO USE 12 GAUGE SHOTGUN TO DISLOGE A PLUG IN T AIR DUCT ON PREHEATER TOWER. HE FIRED GUN @ POINT BLANK RANGE @ THE PLUG. THE SLUG SHATTERED & SMALL PIECES OF LEAD HIT HIM ON R IGHT THUMB & FOREARM. HE WAS SENT TO ER FOR TREATMENT. THEY CLEANED & DRESSED THE WOUNDS - THERE WERE NO SUTURES.
USING HAMMER & CHISEL TO REMOVE MILL BALLS FROM DISCHARGE LINERS WHEN THE CHISEL HIT A BALL & A PIECE OF THE BALL BROKE LOOSE ENTERING THE LEFT HAND.
EMPLOYEE WAS USING A 4' PRY BAR TO MOVE TOE PLATE INTO PLACE TO BE WELDED. THE PRY BAR SLIPPED OFF THE PIECE OF CHANNEL AND STRUCK THE GRATING THE MAN WAS STANDING ON. THE BLOW DISLODGED THE G RATING CAUSING THE MAN TO FALL. THE EMPLOYEE SUSTAINED BRUISING TO HIS LIVER AND SPLEEN, FRACTURES TO BOTH SHOULDER, PELVIS AND TAIL BONE.
PRIED DUST COLLECTOR DOOR OPEN WITH CHANNEL LOCKS TO BREAK THE SEAL, PULLED DOOR OPEN & THEN PULLED BAG OUT. WHEN TRIED TO STAND, FELT PAIN IN BACK. DID NOT REQUEST TO GO TO DOCTOR UNTIL 8/10.
EE WAS RIGGING A 1 TON COME ALONG TO ACCESS DOOR WHICH WAS NEW AND WAS PULLING IT IN PLACE WITH COME ALONG, THEN STOPPED. EE ON OTHER SIDE OF DOOR WITH CROW BAR WORKING THE DOOR BACK OF FORTH WHEN DOOR SLANTED COME ALONG COME LOOSE AND HIT EE IN FOREHEAD AND HARD HAT CAUSING SMALL LACERATION TO FORHEAD TOOK TO CLINIC FOR COUPLE OF SCUTURE AND RETURNED WITH A THE HOUR.
