Employee was removing rock from a spill from a plugged chute when chute was free a rock rolled off from the chute hitting employee in the top of the hard hat. It became a lost time on Aug 16, 2016
(Note: the shift starts 6:00 pm through 6:00 am the following day). While removing a rock jammed between the skirt boards, the rock came loose and the bar moved unexpectedly pinching the employee's finger between the bar and the side skirt board.
Employee was loosing a coupling on a water hose and the wrench slipped striking the employee in the mouth, chipping a tooth.
While using a box cutter knife employee cut left leg, which resulted in 8 stitches.

Employee tried to strike a piece of the steering arm of a haul truck in place with a hammer. Employee missed his target and hit his left middle finger. Examination by a doctor revealed a fracture in his finger.
Employee was using a hammer when he struck his finger. The finger was resting against a piece of angle iron when it was hit. The employee received three stitches in his finger and returned to work the next work day.
Employee was pulling on a handle of a wrench and felt pain in his rib cage. He reported it to his foreman that night, but didn't seek medical attention until March 22, 2004.
EE WAS USING A PRYBAR TO POSITION A PULLEY FOR MAINTENANCE. THE PRYBAR SLIPPED AND THE EE HURT HIS ANKLE. THE EE DID NOT SEEK MEDICAL ATTENTION UNTIL 6/9/03.
