Later that evening after he left work his left eye starting bothering him. He flushed the eye, but continued to have irritation. In the morning he reported it to his supervisor and was sent to the doctor.
The exit conveyor belt on the hydraulic splitter was torn. The employee was trying to splice the belt back together using wire. He was trying to punch a hole through the belt for the wire with a locking blade knife (rather than drilling holes as prescribed). The twisting motion unlocked the blade, and the blade unlocked and resulted in laceration to the right index finger.
