Went to grab a piece of rock and another piece fell on EE's hand.
Splitting rock. Material broke and tried to catch falling rock. ended up pinching hand in between 2 rocks.
EE was moving a rock on the splitter when the material broke smashing EE's hand.
Working splitter and wind blew through overhead door blowing foreign object into associate's eye.
Was Racking a rock off incline of the thin veneer saw and rock rolled onto a finger.

Employee was building pallets in back of fabrication shop. When nailing pieces of wood together with nail gun, nail did not go into wood like planned. Instead it bounced off wood and recoiled the nail where employee was holding the piece of wood, puncturing EE's finger.
Was moving a stone with crowbar. It slipped out from under stone and the edge of the stone landed on the edge of employees hand. EE went to hospital after work but finished working the rest of the day.
Trying to start a fire in the woodstove, resulted in a burn on left hand.

Employee stated that employee grabbed a rock off of the conveyor belt, turn to palatalize the rock and as employee did, employee heard employee's knee pop. Employee's knee/leg then gave out and employee fell to the ground
A block of stone was cut into slabs on our MEM Saw. We flipped the block to cut the back side lips off and then re-loaded it onto the beams to be cut again. EE was lining up the wire for the cut, the block broke and pinned EE's legs against the wall of the MEM. EE was taken directly to the hospital and diagnosed with a compound fracture in left leg.

Miner was splitting a rock and EE's left index finger was caught between the rock and a porn of the splitter. The finger was treated at the local walk in clinic without significant damage done to the miner.

Employee bent over the wire basket to pick up a piece of veneer and when he did, he had a pain go through his lower back.
Supervisor was pulling pin out of guide, that was used as an anchor. Pin was tight causing it to go air born when free. When pin came down it struck employee on the top of his head causing a slight laceration to the crown of the employee's scalp.

Employee was walking on gravel lot outside fabrication building and twisted his left ankle.
Went to Landmart Gas Co. to get a propane tank. Employee helped the supervisor lift a 100lb propane tank and carry to truck. The employee complained of lower back pain, but indicated he could return to work. He continued work, handling stone and buidling sample stone panels. Later in the shift he experienced severe back pain and had to be taken to hospital.
Employee was assisting another operator to reposition a rock slab on a conveyor and strained his right shoulder.

Employee was cutting stone veneer corners with a Target saw. The stone kicked up pushing his right hand thumb into the blade resulting in a laceration.
Employee was building pallets with a nail gun and he laid it on the ground. When he walked around the pallet he kicked the gun with his left foot and apparently loose gravel caused the trigger to depress and fire a nail into his foot.
Employee was using a chipping hammer to chip rough edge off stone product and experienced pain in his right forearm.
Employee was stacking veneer and bumped his knee on the turn table. He refused first aid and medical treatment on the date of the accident. On 2/6/07 he complained of knee pain and requested to go for medical treatment attributing the knee pain to the incident on 1/25/08.

The employee was using a table saw in the maintenance shop to cut wood shims for the target saws in the saw shop. While ripping short pieces lengthwise, the piece jumped and the operator got his left hand fingers in the saw blade causing a laceration on three fingers

The employee complained of pain in the left shoulder and arm due to operating a Teledyne forklift on the yard over a two week period.

The operator was splitting stone and the stone kicked up and smashed left middle finger between stone and splitter head. *E-document #298451.EE had an appt. with the Dr. 6 wks. after he had been released to full duty to check on the status of the fractured finger. The Dr. indicated that he aggravated the condition and that it required further treatment.

The employee was hand stacking rock on a pallet throughout the shift. At the end of the shift the employee complained of his back hurting. He indicated that there was no specific time that the injury occurred, just that his back was hurting.
The exit conveyor belt on the hydraulic splitter was torn. The employee was trying to splice the belt back together using wire. He was trying to punch a hole through the belt for the wire with a locking blade knife (rather than drilling holes as prescribed). The twisting motion unlocked the blade, and the blade unlocked and resulted in laceration to the right index finger.
Operator was outside cleaning up on the yard area, he was stepping down off the forklift to pickup material on the ground. He stepped onto a piece of rock and twisted/sprained his right ankle.
Operator was using a hydraulic splitter to split ends off of rock. While splitting the rock, the rock kicked sideways catching the left middle finger between the rock and the frame of the splitter, cutting the left middle finger.
Driving the loader during the shift. EE said he was not experiencing any pain in the morning at the beginning of the shift. He started experiencing some pain by 9:30 a.m., but did not report this to the supervisor. By 11:15 a.m. he informed the supervisor that his back was hurting and he needed to see a doctor.

EE WAS LOADING STONE ONTO THE CONVEYOR FOR THE THIN VENEER SAW. A STONE FELL OFF THE PALLET HE WAS UNSTACKING ONTO HIS FOOT.
