Employee was moving rock forward on rollers and smashed a finger.

Employee was turning a rock on the turn table when the rock rolled over smashing their fingers

Went to grab a piece of rock and another piece fell on EE's hand.

Splitting rock. Material broke and tried to catch falling rock. ended up pinching hand in between 2 rocks.

EE was moving a rock on the splitter when the material broke smashing EE's hand.

Working splitter and wind blew through overhead door blowing foreign object into associate's eye.

Was Racking a rock off incline of the thin veneer saw and rock rolled onto a finger.

Pulling on a rock to get it ready to split in the splitter. Employee said employee heard a tear in the shoulder area.

Clean out plate flipped up smashing finger between plate and chisel.

Employee was building pallets in back of fabrication shop. When nailing pieces of wood together with nail gun, nail did not go into wood like planned. Instead it bounced off wood and recoiled the nail where employee was holding the piece of wood, puncturing EE's finger.

Was moving a stone with crowbar. It slipped out from under stone and the edge of the stone landed on the edge of employees hand. EE went to hospital after work but finished working the rest of the day.

Trying to start a fire in the woodstove, resulted in a burn on left hand.

Stacking rock when the rock slid smashing left index finger between 2 rocks.

Pulling a rock onto the loader table. The rock slid smashing employees finger between 2 rocks.

Rolling Rock forward on table smashing thumb between 2 rocks.

Using Rock Splitter, Piece of Rock shot out from splitter and hit employee in the stomach.

When chipping a rock, a chip of rock came off hitting EE's left forearm

Picking up rock and another rock fell over smashing finger.

Piece of rock fell off top layer and landed on EE's hand between another rock.

Employee was running tumbler and rock went into bucket. Rock tipped over onto employees foot in front of steel toe where laces are.

When sweeping the floor, employee bumped finger on a metal dumpster.

Leaning over the belt to pick a rock up and the employee felt something pop in the back of the knee.

Employee had a piece of rock fall off pallet and hit EE in the back of EE's leg by EE's Achilles tendon. EE and another EE were working together. Other EE tried to give warning and stop rock from falling.

Using a sledge hammer and the handle cracked, cutting the right hand.

Operator of loader was taking slab of stone off conveyor. Slab shifted on forks, fell off of forks, hit the guard rail. Guard rail fell off and hit operator's foot.

Was walking to EE's car at breaktime. Stepped on black ice, slipped and fell on EE's back.

Had hand sitting by belt guard. Belt grabbed EE's left hand and pulled it under roller of the splitter.

Employee was rolling rock on the splitter and the rock fell off the side of the splitter and hit EE's foot.

Use hand held grinder on stone - saw kicked back and cut EE's hand.

Standard laboring - stacking rock

Using ratchet strap that recoiled when releasing.

EE was using a hammer to correct the stone shape while holding veneer with left hand. While striking with the hammer, EE hit left thumb.

Employee was stacking rock and twisted the wrong way and felt pain.

Slipped on floor that had a small amount of water, near our incline conveyor that exits waste, due to rain. Hurt EE's left knee

EE smashed hand while operating splitter. EE was standing by the splitter afterwards and was very quiet. EE went to step away from the machine and lost consciousness from pain in hand - EE then struck head causing a gash above the right eye.

Chipping stone with a hammer and accidentally smashed EE's fingers between hammer and stone

Employee was picking up small waste rock from floor.

Slipped walking down steps into quarry. All weight shifted onto one leg at an awkward angle.

Employee was stacking rock coming off one conveyor to another. EE right hand got caught between roller guard and belt

Rock broke and smashed employees hand between 2 rocks on Splitter.

Stacking stone and lower back popped

Stacking rocks in pallet and pinched finger between two stones.

Employee stated that employee grabbed a rock off of the conveyor belt, turn to palatalize the rock and as employee did, employee heard employee's knee pop. Employee's knee/leg then gave out and employee fell to the ground

A block of stone was cut into slabs on our MEM Saw. We flipped the block to cut the back side lips off and then re-loaded it onto the beams to be cut again. EE was lining up the wire for the cut, the block broke and pinned EE's legs against the wall of the MEM. EE was taken directly to the hospital and diagnosed with a compound fracture in left leg.

Miner was splitting a rock and EE's left index finger was caught between the rock and a porn of the splitter. The finger was treated at the local walk in clinic without significant damage done to the miner.

Employee bent over the wire basket to pick up a piece of veneer and when he did, he had a pain go through his lower back.

Supervisor was pulling pin out of guide, that was used as an anchor. Pin was tight causing it to go air born when free. When pin came down it struck employee on the top of his head causing a slight laceration to the crown of the employee's scalp.

Operator was stacking stone veneer when he felt pain in his right leg, lower right back and upper left back.

Foreign object flew into right eye while operator was driving a John Deere loader.

Lost balance and fell. while falling, operator reached out with his right hand to break his fall and caught it between the belt and the guard on a conveyor.

While splitting a piece of stone, the stone kicked up and pinched the operator's finger between the stone and the frame of the machine

While operator was performing a splitting operation, a slab of stone fell and caught the operator's left arm causing an abrasion.

While shrink wrapping a pallet of stone the employee bent over and pulled a muscle in his lower back.

While moving a pallet of stone slabs ,operator extended forks on the Teledyne fork lift to set pallet on floor.In doing so the machine tilted forward and operator hit his head on the roll cage causing a laceration above left eye.

In the process of replacing a conveyor belt, the employee was pulling the new belt into place and strained his lower back.

EMPLOYEE TRIPPED,AND WHILE TRYING TO BREAK HIS FALL,GRABBED THE WALL WHERE A NAIL WAS EXPOSED AND CUT HIS HAND ON THE WALL

Unloading slabs of stone from saw by hand to a pallet and caught his fingers between two slabs. Pinched tips of 3rd and 4th finger of left hand. Mangled tip of 3rd finger. No broken bones.

Employee was walking on gravel lot outside fabrication building and twisted his left ankle.

Went to Landmart Gas Co. to get a propane tank. Employee helped the supervisor lift a 100lb propane tank and carry to truck. The employee complained of lower back pain, but indicated he could return to work. He continued work, handling stone and buidling sample stone panels. Later in the shift he experienced severe back pain and had to be taken to hospital.

Employee was assisting another operator to reposition a rock slab on a conveyor and strained his right shoulder.

Employee was cutting stone veneer corners with a Target saw. The stone kicked up pushing his right hand thumb into the blade resulting in a laceration.

Employee was building pallets with a nail gun and he laid it on the ground. When he walked around the pallet he kicked the gun with his left foot and apparently loose gravel caused the trigger to depress and fire a nail into his foot.

Employee was using a chipping hammer to chip rough edge off stone product and experienced pain in his right forearm.

Employee was stacking veneer and bumped his knee on the turn table. He refused first aid and medical treatment on the date of the accident. On 2/6/07 he complained of knee pain and requested to go for medical treatment attributing the knee pain to the incident on 1/25/08.

While performing the morning stretching exercises, the operator twisted his back and strained his back.

Operator smashed finger when stacking rock on a pallet

Operator was cutting stone on the target saw and a rock chip got behind his glasses and into his left eye.

Walking around the work area, operator stepped on a rock on the floor and twisted his right ankle.

Stacking stone and smashed fingers on right hand between stones.

Employee was turning over a slab and caught his right index finger between two slabs resulting in a cut.

Employee was lifting a piece of stone off the conveyor belt and felt a pain in his lower back.

Two employees were replacing the conveyor belt with metal lacing. One was hammering the metal lace and a piece broke off and embedded in the other employee's right forearm.

Employee was chipping rock with a hammer and felt pain in his right forearm.

Employee was climbing down out of the Teledyne forklift and twisted his back when stepping down.

Employee had come inside at the end of the shift to clock out. The day was hot and he felt ill and passed out briefly. He hit his head on the floor when he collapsed. He was transported to the hospital in an ambulance for evaluation/treatment.

Employee was cutting stone with the Target saw. The stone flipped over while cutting, smashing his right index finger.

Employee was chipping edge off a piece of stone with hammer. Rock chips struck his face. The employee was wearing safety glasses. He did not feel anything go into his eye. Later that evening after he left work his left eye starting bothering him. He flushed the eye, but continued to have irritation. In the morning he reported it to his supervisor and was sent to the doctor.

The employee was using a table saw in the maintenance shop to cut wood shims for the target saws in the saw shop. While ripping short pieces lengthwise, the piece jumped and the operator got his left hand fingers in the saw blade causing a laceration on three fingers

Employee was stacking rock on pallets at end of conveyor. While picking up a stone he experienced pain in his right shoulder and neck.

Employee has right elbow soreness from cleaning silt from the floor drains.

The employee complained of pain in the left shoulder and arm due to operating a Teledyne forklift on the yard over a two week period.

Employee was positioning stone on the feed conveyor to the hydraulic splitter. He was pushing/pulling on the stone and experienced left shoulder pain.

Employee reported the injury to the supervisor on 10/10/06. Employee complained of lower back pain from stacking stone the previous day.

Positioning stone blocks on feed conveyor to the splitter. Employee complained of lower back pain several hours into the shift.

Employee was unloading and palletizing stone off the exit conveyor of the TXS saw. He also had to knock off the rough edge of some of the stone coming out of the saw. He struck the rough edge of the stone against the metal conveyor frame to break off the rough edge and a piece went under the safety glasses and into his right eye.

While stacking stone on a pallet in the outside yard the employee picked up a stone. It slipped in his right hand and caused a laceration to his pinky and ring finger.

Employee was feeding rock on the conveyor to the TXS saw. He was experiencing pain in his left hip.

While stacking rock, the rock slipped and dropped on his right hand, smashing his 4th finger.

Accident was reported on 7/18/06.Employee indicated that he had dropped a rock on his ankle when stacking rock. His left ankle was swollen and bruised.

The operator was splitting stone and the stone kicked up and smashed left middle finger between stone and splitter head. *E-document #298451.EE had an appt. with the Dr. 6 wks. after he had been released to full duty to check on the status of the fractured finger. The Dr. indicated that he aggravated the condition and that it required further treatment.

The employee was hand stacking rock on a pallet throughout the shift. At the end of the shift the employee complained of his back hurting. He indicated that there was no specific time that the injury occurred, just that his back was hurting.

The exit conveyor belt on the hydraulic splitter was torn. The employee was trying to splice the belt back together using wire. He was trying to punch a hole through the belt for the wire with a locking blade knife (rather than drilling holes as prescribed). The twisting motion unlocked the blade, and the blade unlocked and resulted in laceration to the right index finger.

Operator was outside cleaning up on the yard area, he was stepping down off the forklift to pickup material on the ground. He stepped onto a piece of rock and twisted/sprained his right ankle.

Operator was using a hydraulic splitter to split ends off of rock. While splitting the rock, the rock kicked sideways catching the left middle finger between the rock and the frame of the splitter, cutting the left middle finger.

Driving the loader during the shift. EE said he was not experiencing any pain in the morning at the beginning of the shift. He started experiencing some pain by 9:30 a.m., but did not report this to the supervisor. By 11:15 a.m. he informed the supervisor that his back was hurting and he needed to see a doctor.

EE WAS LOADING ROCK ONTO THE FEED CONVEYOR FOR THE THIN VENEER SAW. HE COMPLAINED OF PAIN IN HIS BACK.

TWO WORKERS WERE LOADING A SLAB OF STONE ON THE EQUIPMENT. INJURED PULLED A MUSCLE IN SHOULDER.

EE WAS LOADING STONE ONTO THE CONVEYOR FOR THE THIN VENEER SAW. A STONE FELL OFF THE PALLET HE WAS UNSTACKING ONTO HIS FOOT.
