Employee was aligning a beam to a column using a bull pin and small sledgehammer. Went to strike the bull pin and missed causing EE's hand to get caught between the column and sledgehammer handle. Sustained a fracture and laceration to the right index finger.

EE was performing maintenance work with a colleague at the primary plant. Using a hammer, the colleague intended to strike a hammer wrench held by the EE but missed, bouncing off the feeder wall and striking the EE's right ring finger.

EE was loosening rubber skirting for a belt. EE was loosening a nut the wrench lost hold of the bolt and EE's hand slipped causing the bottom part of the wrench to make contact with the running belt. Once the wrench contacted the belt, the wrench pinched/pinned EE's right middle finger to the metal brace which holds the rubber skirting in place.

EE was working on the clinker breaker. They were trying to remove the clinker breaker hood when D ring that the comalong was attached to broke off and struck them on the left leg. There were supervisors on site and one was right next to them. EE had just turned around when struck on the left leg. EE was taken to the hospital and was determined they had an open fracture.

EE was using hand & a metal ruler to remove a broken soap dispenser mounted with adhesive to a glass mirror. While prying the dispenser away from the mirror, the glass broke & cut through the EE's Level 5 cut resistant gloves ee was wearing. This resulted in a laceration to the two middle knuckles. EE received 4 stitches & was released to full duty with no restrictions.

Employees used a come-along to raise the belt off of the idle roller to take the pressure off and allow for easy change of the roller. Upon raising the belt, the employee found self under the load. The come-along slipped off the frame of the structure allowing the belt to forcibly drop down and strike the employee on the top of the head.

Miner was closing the slag slide on the tower. After 3-4 strikes, the miner's wrist began hurting. Gave employee some ice and refrained from using for the rest of the shift. That morning, EE went to the doctor and was treated as a first aid, wrist sprain. On 4/2/18 EE received a repeat Rx, which created the reportable injury status.

EE was attempting to cut a Tie Strap from the frame of the glass on the wing window (not emergency exit window) of a CAT 992G Front End Loader with a pocket knife. As EE began to cut, EE's thumb touched the lock on the knife, causing the blade to close on hand. EE received 5 stitches and was released to full duty with no restrictions.

While attempting to break a plug on a wheel loader at work, the employee slammed hand between the ratchet wrench and the inside of a steel rim detaching the whole fingernail and causing a laceration to the left pinky finger.

EE was removing a bushing. A chisel was used to create a gap between the cross shaft and bushing. EE pulled the bushing (chisel still wedged in assembly) off the cross shaft. EE tossed the bushing 4'6" to the side. The chisel dislodged from bushing & projected upward 7'6" at a 45 degree angle back towards EE. The chisel struck EE in the lip under the right nostril.

EE was in bottom deck of shaker removing a knock-in strip with a 6lb hammer. With hammer in right hand, he swung to hit the knock-in strip. His (R)hand slipped down the handle causing him to miss his target and strike his (L)thumb that was placed approx 6" above target. This caused a laceration requiring stitches. EE returned to work with restrictions of no gripping with thumb.

Employee was attempting to remove a chunk of material caught between a chute and a conveyor with a pry bar. The bar slipped off the chunk contacting the belt this caused the bar to bounce back striking the employee behind his left ear causing a contusion and stiff neck.

Maintenance crew ee and I were attempting to pull the counter weight off the timing side of a Telsmith screen shaker. We had a puller and two come a longs on it as well as heating it. it started coming off as we cranked on the come a long when all of a sudden it broke loose and flew off resulting in smashing his hand between the counter weight and I beam.

The employee was using a pocket knife to trim the excess off of a zip-tie that was securing a grease hose to the framework of a conveyor. He took a step forward and lost his balance, this caused him to fall forward. The employee reached up with both hands to break his fall. While doing this the knife blade came into contact with his left thumb.

EE was adjusting the tension on a 1.5 Ton come-a-long from a gravity take-up system, when the pin that holds the latch broke releasing the chain, allowing the chain to swing back striking EE on the arm. He may possibly have a hairline fracture near his elbow.

As the maintenance crew was attempting to unclog the sand line, the unsecured pipe used for the come-along slipped off the pump side, striking the employee on the back of right shoulder. As this happened, the come-along slipped off the pipe, causing the pipe to come back up and strike the employee across the nose and forhead.

Injured EE and another EE were repairing Dredge pipeline. The other EE was hammering bolts into the flanges of the pipeline. Injured EE reached down to push one of the bolts in, thinking the other EE was finished hammering, but his left middle finger was struck by the hammer. Injured EE was taken to the Doctor for evaluation.

On 8-20-2010, employee was working on a vertical turbine water pump to free the clutch. He was using a 24" pipe wrench to break free the back stop. The shaft broke free and spun the pipe wrench and struck his hand. He was taken to the doctor for x-rays to check for fractures. The x-rays were negative for fractures but the small finger was bruised severely.

The employee was using a utility knife to trim the edge of a urethane screen so he could get it set into place. While in the process of trimming this screen, the utility knife slipped off the screen and cut through his glove into the bottom of his thumb. This resulted in a 2 inch laceration that required 10 staples to close.

Unloading a pallet with a chain hoist the load shifted, the person helping unload the pallet let go of the pallet & employee's left hand was caught in the chain causing severe bruising and a break in the hand. Due to severe bruising of the hand the first xray did not show a break. Second set of xrays taken on 5/22 found a break which was visible due to reduction of swelling.

Employee was cutting crown bar rubber at the 2-deck shaker (cutting excess off), as he was cutting the blade cut his knuckle on his left thumb. We took him to the clinic for medical attention, the doctor put stitches due to the location of the cut not the depth.

Three workers were measuring stock piles at the sand plant. They were using a hundred foot vinyl measuring tape with an inch and half nut secured on the end for weight. The weight is slung up on top of the stock pile to measure the slope. While slinging the tape, employee heard and felt a tear in his right shoulder.

Employee was using Hi-Lift jack to level Mega-Stand that is used to lift aluminum dome. Employee did not lower the handle enough for safety latch to engage stop blocks. Employee took his hand off the jack handle and was raising his head, when the handle came back up to upright position, striking employee in the lower lip. Employee received laceration to lower lip.

A hand chain hoist was being used to raise the bottom side of belting on a conveyor that return idlers were being added. While placing his left hand under the belt, as to receive one side of the idler, the hoist slipped causing the belt to come down on his hand. He received a contusion / sprain to his left wrist and hand area.
