EE was opening a box of welding rods using a self-retracting blade box cutter. While cutting the thick protective plastic covering, EE cut in a direction toward self. During the cutting motion, the knife got caught in the plastic, it became free the pulling motion caused a laceration to the middle joint (knuckle) of EE's left thumb.

While using a pry bar to release a stuck open-top railcar gate latch, employee felt a pop in EE's right shoulder, resulting in medical treatment and work restrictions for a suspected torn rotator cuff.

Small finger on the right hand was crushed by steel bar. The utility operator was dislodging a plug and bar caught the rotor feeder slamming his hand against the edge of the hopper. No stitches, no broken bones, just prescription for pain and an antibiotic. No loss time. Returns to work next shift.

Employee was drifting a screw conveyor into place with use of a come-long, employee reached down and pushed on the screw to help guide it into place, the screw shifted position, employees right pinky finger got caught between the chain of the come-a-long and the screw. Employee was taken to the hospital , eight stitches were inserted into his right pinky finger.

EE was attempting to open a lid on a 5 gallon bucket. He was cutting the seal for the lid off, pulling downward and toward his body, EE was kneeling at the time. the knife slipped off the plastic seal and punctured his right calf.

AS HE WAS WORKING ON THE FROZEN MATERIAL THE CHUNK BROKE INTO TWO PIECES AND SHIFTED. ONE PIECE STRUCK HIS LEG, THE FORCE OF THE BLOW HYPEREXTENDING HIS KNEE. A DOCTOR DIAGNOSED KNEE STRAIN & GAVE MEDICATION.

EE WAS IN K-1 COOLER UTILIZING THE GUZZLER TRUCK FOR CLEAN UP PURPOSES. THE GUZZLER HOSE BECAME PLUGGED, HE WENT TO UNPLUGG THE HOSE, IT WAS DETERMINED THAT THE HOSE NEEDED CUT. SO HE POSITION ED THE HOSE ON HIS LEG AND BEGAN CUTTING WITH A UTILITY KNIFE. THE UTILITY KNIFE WENT THROUGHT THE HOSE INTO HIS LEG RESULTING IN 4 STITCHES TO RIGHT THIGH.
