Strain/Sprain to right shoulder while climbing into a 980 front end loader.
While climbing up the ladder on 415 loader, at the 2nd rung, Employee missed grabbing the handrail with his left hand. As a result, he swung backward and to the right, hitting the left-rear loader tire with his back and went to the ground. Upon landing, he hit the ground with his left palm and right elbow.

EE INFO RMED DOCTOR THAT INJURY OCCURRED AT WORK. DOCTOR REMOVED A METAL PARTICLE & PRESCRIBED MEDICATION. MCC CONTESTED CASE AS TO WHETHER INJURY OCCURRED AT WORK. WTO W/C DETERMINED ON 8/4/03 THIS

EMPLOYEE WAS CUTTING RUBBER BELTING MATERIAL WITH A RAZOR BLADE KNIFE PULLING BLADE ACROSS THE MATERIAL. AS EMPLOYEE WAS FINISHING HIS CUT THE BLADE SLIPPED OUT OF THE BELTING MATERIAL AND TH E EMPLOYEE CUT HIS LEG BONE AND RIGHT KNEE. LACERATION WAS CLEANED AND CLOSED WITH SUTURES.
