Fall from machine

While climbing up the ladder on 415 loader, at the 2nd rung, Employee missed grabbing the handrail with his left hand. As a result, he swung backward and to the right, hitting the left-rear loader tire with his back and went to the ground. Upon landing, he hit the ground with his left palm and right elbow.

EE HAD A PIECE OF METAL IN HIS EYE. EE DID NOT INFORM HIS SUPERVISOR OR COMPLETE AN INJURY REPORT. ON A LATER DATE HIS EYE WAS IRRITATED & HE WENT TO THE HOSPITAL TO HAVE EYE EXAMINED. EE INFO RMED DOCTOR THAT INJURY OCCURRED AT WORK. DOCTOR REMOVED A METAL PARTICLE & PRESCRIBED MEDICATION. MCC CONTESTED CASE AS TO WHETHER INJURY OCCURRED AT WORK. WTO W/C DETERMINED ON 8/4/03 THIS

EMPLOYEE WAS CUTTING RUBBER BELTING MATERIAL WITH A RAZOR BLADE KNIFE PULLING BLADE ACROSS THE MATERIAL. AS EMPLOYEE WAS FINISHING HIS CUT THE BLADE SLIPPED OUT OF THE BELTING MATERIAL AND TH E EMPLOYEE CUT HIS LEG BONE AND RIGHT KNEE. LACERATION WAS CLEANED AND CLOSED WITH SUTURES.

THE EE WAS DESCENDING HIS HAUL TRK WHEN HIS FOOTING MISSES THE LAST STEP (LADDER RUNG) AND HE FELL LANDING ON HIS BACK. THE EE SUSTAINED A SORE BACK. THE EE WAS SEEN BY A PHYSICIAN & PLACED ON 2 DAYS OF RESTRICTED DUTY.
