EMPL HAD USED TORCH TO CUT HEAD OFF OF BOLT. HE REMOVED CUTTING GOGGLES THEN USED HAMMER TO CHIP AWAY BURNT METAL. FLAT METAL WASHER STRUCK HIM IN RIGHT EYE AREA.
