EE was hitting a stuck drill head hammer with a pin hammer and a brass hammer. A piece of the drill head hammer metal fractured off, and entered EE's left leg on EE's inner thigh between EE's knee and groin. Doctor's wanted to wait a few days before performing the removal of the metal, in order to have a good OR room for the removal.

EE was trimming conveyor belt sections in preparation for a job when EE's hands slipped and EE's utility blade malfunctioned releasing the blade. EE sustained a small puncture injury to EE's left palm. EE was taken for medical evaluation where EE received 4 stitches and was released to work without restrictions.

EE was driving a wedge behind a ball mill liner to remove it when EE accidentally struck EE's left hand with a hammer. EE was taken for medical evaluation and diagnosed with a contusion. X-ray was negative for fractures. EE was on restricted duty for 4 days.

Contractor EE was stripping wire at the MAAG MCC when knife slipped and cut right index finger. EE was taken for medical evaluation where EE was diagnosed with a laceration to the right index finger, given 8 stitches and returned to duty with restrictions. EE returned to full duty on 05/03/2018.

EE was hammering out a jib pin when a piece of foreign material (presumably paint) went under safety glasses and into left eye. EE was sent for evaluation and diagnosed with a scratch to the cornea of left eye. No material was found or removed from eye and EE was released with no restrictions. EE was issued prescription eye drops making this event reportable.

On 7/18/16 EE filed a F/A report stating that EE strained EE's left shoulder while using a pole hook to align a loading spout into a bulk trailer. EE declined to be medically evaluated at that time. On 7/28/16 EE requested medical evaluation and was sent to Occ Med. EE was diagnosed with a muscle strain and returned to duty with restrictions making this case reportable.

EE was tightening a bolt with a 3/4" ratchet when he felt a "pop or pull" on the right hand side of his chest around the lower ribs. EE was sent for medical evaluation and returned to work with restrictions. Diagnosis pending.

EE was removing an oil drain plug on a gear box when his allen wrench broke and he fell backwards into a grease fitting which struck him in his back just below the right shoulder. EE was diagnosed with a contusion/ myalgia and released to work with restrictions.

EE was using a box knife to cut a hose and the knife slipped in his hand causing a laceration to the tip (pad) of his right index finger. EE received 6 stitches and was released to work with no restrictions. EE was not wearing gloves at time of incident.

EE was changing an air seal on a pump and using a piece of round stock and hammer to rotate the shaft collar into position. The round stock dislodged after being struck and hit the EE in the lower lip causing a laceration. EE was taken for medical evaluation, received 1 (one) suture and released to work with no restrictions.

EE was striking a slide gate with a hammer and he struck the tip of his left pinky finger with the hammer. EE reported the injury as a first aid on 10Oct12 and sought medical evaluation on 11Oct12. EE was diagnosed with a subdural hematoma and fracture to the tip of his left pinky and released to work with restrictions.

When tearing out refractory a spring on the rivet buster broke. Employee after being told not to use lower exit climbed down the ladder, knocked off the rivet buster from the deck hitting himself on the upper shoulder and back.

EE was pulling on a big wrench to tighten a nut when he felt a pop in his left shoulder. He initially reported it as only a first aid and did not think he needed medical attention. The following day he had pain and swelling in his left shoulder and was taken to the company doctor in the morning. He was diagnosed with an "strain/impingement", given meds and placed on light duty.

EE WAS STANDING ON TOP OF CRUSHER HALF TO TIGHTEN FLANGE BOLT. HE WAS PULLING ON THE WRENCH AND IT SLIPPED OFF THE NUT. EE FELL DOWN OFF THE TOP OF CRUSHER HALF. HE LANDED ON HIS LEFT HAND WHEN HE TRIED TO BREAK HIS FALL.

EMPLOYEE WAS CLEANING OUT A RAIL CAR. THE MATERIAL WAS STUCK IN THE HOPPERS AT THE BOTTOM OF THE CAR. HE USED A SLEDGE HAMMER TO TRY TO CLEAN THE MATERIAL OUT AND WHILE DOING THIS HE FELT A STRAIN IN HIS BACK.
