EE was removing a metal panel from the side of a machine for maintenance. EE had EE's hand on the bottom of the panel when EE removed the top bolt. The panel slipped, pinching EE's fingers between the panel and the table.

The employee was using a crowbar to move a rock when it broke on a natural seam in the stone and struck EE on the right arm.

Employee was using splitting maul to split rock. The tip chipped off striking employee on the right side of chest by the collar bone.

Employee was in the splitting bay using a trimming hammer and a small piece of stone flew off and wedged under employee's skin.

Employee was chiseling cap of rock when a piece of the chisel head broke and went into EE's left bicep.

Piece of metal off a splitting trimmer went through his glove and stuck in his index nail.

The employee came in to work on May 28th, 2014 and stated that he had a sore back from splitting stone.

Employee was splitting stone. As the hammer struck the stone he rolled his wrist, re-injuring a pre-existing condition.

He was trying to pry a stone apart with a pry bar and it hit him in the head.

He was using a chisel to trim stone and a piece of the chisel flew off into his wrist. He had no pain when it happened so he didn't report it right away. It started hurting on 8/9/11 and he reported it then.

He was trimming a piece of rock when a small chip of steel from the hammer hit him in the right side of the neck causing a puncture wound.

Employee was splitting stone, using a 16lb stone splitting hammer, when a piece of steel chipped off the hammer head and struck him in the arm. The small pc of steel punctured his arm and has to be removed.

Employee works splitting stone with a mason hammer. While trimming stone a piece of steel flew off hammer and penetrated his face.

EMPLOYEE WAS SPLITTING MATERIAL USING A 6 LB HAMMER WHEN HE HIT THE CORNER OF THE HAMMER ON THE STONE CAUSING A SMALL PIECE OF STEEL TO FLY OFF HAMMER. THE SMALL PIECE OF STEEL ENDED UP EMBEDD ED IN THE SKIN OF THE CHEST AREA.

EMPLOYEE WAS SPLITTING STONE WITH A 6 POUND TRIMMER WHEN HE HIT HIS LEFT THUMB ON MISTAKE. EE SUFFERED A FRACTURE ON THE TIP OF LEFT THUMB.

EE WAS USING A PRY BAR TO SEPARATE DRILLED SLABS WHEN HE SLIPPED OUT OF THE DRILL HOLE & FELL DOWN HITTING HIS LOWER BACK. EE SUFFERED A BACK SPRAIN & BRUISING.

EMPLOYEE WAS SWINGING A 16 LB HAMMER ATTEMPTING TO SPLIT A PIECE OF STONE WHEN THE HAMMER BOUNCED OFF THE STONE AND HIT THE EMPLOYEE'S RIGHT ANKLE.

WHILE SPLITTING A PIECE OF STONE WITH A TRIMMING HAMMER AND A CHISEL THE TRIMMING HAMMER SLIPPED OFF THE CHISEL AND EE HIT HIS LEFT THUMB.
