EMPLOYEE STEPPED OM WELDER CORD AND ROLLED OUT FROM UNDER EMPLOYEE'S FOOT CAUSING THE EMPLOYEE TO FALL. EMPLOYEE FIRST WENT TO THE DOCTOR ON 2-28-00 REGARDING THIS INJURY.
