EE was moving a stone when the edge of the stone broke and caused a laceration to EE's left arm.

EE was rushing to get a task completed and slipped hitting left hand on a grate in the floor causing a laceration.

EE was removing a metal panel from the side of a machine for maintenance. EE had EE's hand on the bottom of the panel when EE removed the top bolt. The panel slipped, pinching EE's fingers between the panel and the table.

Employee reached into an unguarded exhaust fan to open the cover, causing fingers to strike fan blades.

Employee stepped off a truck ladder and tripped in a hole, falling backwards and hitting his arm on the truck parked next to him. Employee received a laceration to his left bicep.

Employee was attempting to move a piece of material(rubble stone) on a skid when an adjacent piece fell over cutting her knee. Laceration to right knee requiring stitches was result of incident.

EMPLOYEE WAS PALLETIZING MATERAIL (CAPSTONES) WHEN A PIECE TIPPED OVER & PINCHED HIS LEG AGAINST THE PALLET. EE REPORTED BRUISING & SOME SWELLING BUT CONTINUED TO WORK. AFTER 2 WEEKS HE WAS COMPLAINING OF PAIN & DECIDED TO GO TO THE DOCTOR. DR DID X-RAYS & FOUND A FRACTURE IN THE TIBIA. 1ST VISIT TO DR ON 4-23-04. EE WORKED FROM 4-9-04 TO 4-23-04.

EE WAS TAILING (WORKING @ END OF) GUILLOTINE OUTFEED WHEN A PIECE OF MATERIAL FELL OFF PINCHING HIS RT.INDEX FINGER. INJURY WAS A LACERATION THAT RESULTED IN STITCHES.

EE WAS REMOVING A GUARD FOR MAINTENANCE RELATED ACTIVITIES WHEN IT FELL IT AND HIT HIM IN THE HEAD. EE RECEIVED A BRUISE AND CONTINUED TO WORK. EE LATER ALLEGES HE WAS EXPERIECING HEADACHES AN D MISSED SOME TIME AND WENT TO DR. EE CONTINUED TO BE SPIRATIC WITH ATTENDANCE. DR NOTIES INDICATED MENTAL ISSUE NEW A PRIOR TO WORKING WITH US.

EMPLOYEE HAS BEEN DIAGNOSED WITH A HERNIA. EMPLOYEE ALLEGES HE BEGAN EXP PAIN IN STOMACH AREA AT MINE SITE AND REPORTED IT ON ABOVE DATE. EMPLOYEE HAS HAD PREVIOUS SURGERY IN THE AREA WHERE HE RNIA IS. NO PLANS FOR SURGERY AT THIS TIME.

EE WAS PACKAGING MATERIAL WHEN ONE PIECE SLID DOWN & PINCHED HIS FINGER BETWEEN ANOTHER PIECE. MATERIAL RANGES IN SIZE & WT. APPROX 10-40# +/-. EMPLOYEE RECEIVED AN OPEN WOUND & A FRACTURE TO THE DISTAL END OF LEFT RING FINGER. *EE IS ON LIGHT DUTY ALLOWING TIME FOR HEALING.*

EE WAS DRILLING MATERIAL USING A PNEUMATIC DRILL WHEN HE ALLEGED THAT THE DRILL BIT SLIPPED OUT OF THE DRILL HOLE & HE TWISTED HIS RIGHT SHOULDER.

EMPLOYEE WAS SPLITTING STONE WITH A 6 POUND TRIMMER WHEN HE HIT HIS LEFT THUMB ON MISTAKE. EE SUFFERED A FRACTURE ON THE TIP OF LEFT THUMB.

EE WAS USING A PRY BAR TO SEPARATE DRILLED SLABS WHEN HE SLIPPED OUT OF THE DRILL HOLE & FELL DOWN HITTING HIS LOWER BACK. EE SUFFERED A BACK SPRAIN & BRUISING.

EE WAS CHANGING A DRILL BIT ON A PNEUMATIC DRILLWHEN IT ""POPPED"" UP AND HIT HIM IN THE MOUTH.

EE WAS ROLLING A PIECE OF STONE OFF A CONVEYOR BELT INTO A DUMPSTER WHEN HE FELT A PAIN IN HIS BACK.

EE WAS WALKING TO HER CAR AT THE END OF THE DAY TO WARM IT UP AND SLIPPED ON THE SNOW.

EMPLOYEE WALKED BY A DRILL PRESS AND BRUSHED THEDRILL PRESS CAUSING A STEEL PIN TO ROLL OFF AND LAND ON HER TOE.

EMPLOYEE WAS PLACING A PIECE OF STONE INTO A DUMPSTER AND WHILE DOING SO HE LOST HIS BALANCE AND SLIPPED CAUSING HIM TO LOSE CONTROL OF THE PIECE PF STONE HE HAD AND HIS FINGER GOT CAUGHT BETW EEN THE PIECE OF STONE AND THE DUMPSTER.

EE WAS CARRYING A PIECE OF STONE TO PUT IT INTO A DUMPSTER AND TRIPPED OVER ANOTHER PIECE OF STONE AT WHICH TIME HE FELL CATCHING HIS LEFT RING FINGER BETWEEN THE STONE AND DUMPSTER.

EMPLOYEE WAS SWINGING A 16 LB HAMMER ATTEMPTING TO SPLIT A PIECE OF STONE WHEN THE HAMMER BOUNCED OFF THE STONE AND HIT THE EMPLOYEE'S RIGHT ANKLE.

EE STATES HE WAS LIFTING A PIECE OF STONE FROM THE GUILLOTINE BELT HE FELT A PAIN IN HIS LOWER BACK

EMPLOYEE STATES HE WA PUSHING A STONE ON THE STONE CUTTING MACHINE ROLLS TO PREPARE A STONE TO BE CUT AND FELT A PULL IN THE LEFT SIDE OF HIS STOMACH.

EMPLOYEE STEPPED OM WELDER CORD AND ROLLED OUT FROM UNDER EMPLOYEE'S FOOT CAUSING THE EMPLOYEE TO FALL. EMPLOYEE FIRST WENT TO THE DOCTOR ON 2-28-00 REGARDING THIS INJURY.
