Employee was moving stone when they got their middle left finger caught between 2 stones.

A pipe fitting on a water tank came loose and hit employee on left knee cap.

Employee slipped on ice and injured left leg.

Employee was moving stone when EE pinched EE's right middle finger.

Employee was using a bench grinder when EE's shirt became entangled.

Employee sustained an injury to the left index finger. *Hydraulic tank cover fell on finger.

Employee was cutting stone when a 12in. piece broke off and struck in the face

Employee was using splitting maul to split rock. The tip chipped off striking employee on the right side of chest by the collar bone.

Employee was in the splitting bay using a trimming hammer and a small piece of stone flew off and wedged under employee's skin.

Stone came off the belt and hit employee's right thumb

Employee was lifting stone and pulled muscles in his lower abdomen

Piece of metal off a splitting trimmer went through his glove and stuck in his index nail.

He was using a chisel to trim stone and a piece of the chisel flew off into his wrist. He had no pain when it happened so he didn't report it right away. It started hurting on 8/9/11 and he reported it then.

Employee was lifting stone and pinched his left ring finger.

EE WAS WALKING TO HIS VEHICLE WHEN HE SLIPPED/TRIPPED & FELL. WHEN EE FELL HE CUT HIS ARM.

EMPLOYEE WAS SPLITTING MATERIAL USING A 6 LB HAMMER WHEN HE HIT THE CORNER OF THE HAMMER ON THE STONE CAUSING A SMALL PIECE OF STEEL TO FLY OFF HAMMER. THE SMALL PIECE OF STEEL ENDED UP EMBEDD ED IN THE SKIN OF THE CHEST AREA.

EE WAS LIFTING A PIECE OF STONE FROM THE GROUND TO THE DUMPSTER AND FEL6T PAIN IN LOWER BACK WHILE DOING SO.

EE WAS LIFTING A PIECE OF STONE ON TO A PALLET.

EMPLOYEE WAS LIFTING A PIECE OF STONE WITH ANOTHER EMPLOYEE. THE STONE SLIPPED DOWN AND PINCHED EMPLOYEE'S FINGER.

INJURED EE WAS PICKING UP A PIECE OF STONE WITH ANOTHER EE AND THE STONE SLIPPED OUT OF THE OTHER EE'S HANDS CAUSING THE STONE TO FALL AND CUT THE INJURED EE'S LEFT MIDDLE FINGER.

WHILE SPLITTING A PIECE OF STONE WITH A TRIMMING HAMMER AND A CHISEL THE TRIMMING HAMMER SLIPPED OFF THE CHISEL AND EE HIT HIS LEFT THUMB.
