Employee was assisting cutting plastic ANFO loading hose to length to replace worn hoses on powder truck. While cutting hose, employee-trainee lost control of knife being used to cut pipe, which caused knife to close onto employee's right middle finger, lacerating the skin at/around the first knuckle.

A contractor maintainer was using a 2-pound hammer. An approximately 1.3 cm wedge shaped shard of the hammer head separated causing projectiles that struck the maintainer through the face shield into the lower lip and chipping a tooth. A second shard struck the contractors right hand causing a laceration. The lip required sutures.

employee was cleaning out build up material from hopper with a bar when EE hit EE's elbow on metal hopper causing lateral epicondylitis and contusion, Employee was not put on restrictions until doctors visit on 03/23/2018.

EE was working on unjamming buckets at the loadout chute. He took his gloves off to remove the bolts holding the outside skirt so that he could get to the buckets. Several buckets were stuck together so he hit them with a hammer. When the buckets came apart, they smashed his fingers between the buckets resulting in a laceration to one finger and contusion to another.

Employee was changing a screen on the 90 hammer screen when his wrench slipped and struck the tip of his left index finger. The impact forced his index finger down on the edge of a screen rail. Employee reported to his supervisor and was taken for evaluation. The laceration on his finger was closed using glue and the employee was given restrictions.

Employee was attempting to tighten a 4" pipe coupling in a vise. As he put pressure on the handles to tighten the vise the coupling slipped. This allowed the vise handle to free wheel causing employee's hands to slip and loose his grip. The sudden jerking motion injured his shoulder.

Maintenance repairs/replacing hammers on crusher. A 16 lb. sledge hammer was being used to hammer out a broken hammer bolt. EE was taking a measurement to see how much more hammering was required. Team member assisting in repairs did not notice EE's hand in the way as he continued to hammer on the broken bolt striking EE's left pinky with the sledge hammer.

Employee was lowering a chute with a hand crank device. He lost control of the crank sending it into a freespinning motion contacting his face. This caused a broken nose, fractures to bones in the face, and lacerations.

A mechanic was driving a pin into a Cat loader bucket arm with a sledge hammer. The injured mechanic was holding the arm when the sledge hammer glanced off the pin striking his hand.

Ballmill door was to be opened for repairs. the door is sealed so that material does not excape. Wedge are driven into the door flange to break the seal and then a come-along is used to lift the door to an open position. EE croched next to the wedge to remove it and it dislodged striking ee on the knee. No equipment damamge.

EE WAS CUTTING SOME SKURTBOARD FOR THE SCREENS AT THE AGG PLANT. HIS KNIFE SLIPPED FROM HIS HAND AND CUT A WOUND ON THE INSIDE OF HIS LEFT FOREARM. THIS REQUIRED FOUR SUTURES TO CLOSE THE WOUND. THE DOCTOR RELEASED HIM TO WORK THAT SAME NIGHT.

THE INNER CAGE MILL HAMMERS AND CAGE WERE BEING ASSEMBLED. SMALL WOODEN BLOCKS WERE BEING PLACED UNDER EACH BOLT TO SUPPORT IT IN THE RECESSED POSITION. AS THE BAR WAS BEING USED TO RAISE THE RING, PRY BAR SLIPPED AND THE RING DROPPED APPROX 2 INCHES MASHING THE TIP OF THE 4TH FINGER. FINGER TIP BONE WAS FRACTURED.
