Over-exertion in pulling or pushing objects

As an employee was attempting to climb into the back of a cargo trailer, their shoulder dislocated.

Struck against a moving object

Employee was using a Front End Loader to feed material to the plants. As EE backed up to get another load of material, the employee backed into another parked front end loader. The employee received a work restriction due to a muscle strain to EE's neck and right shoulder from the impact. Employee is due back at the doctor on 1/17/2025.

Struck by falling object

Employees were reinstalling the belly pan on the undercarriage of the dozer. The rigging on the belly pan slipped allowing it to swing back down and pin the mechanic. The mechanic was diagnosed with three fractured ribs. Mechanic received a restricted work designation to lift no more than 20 lbs. and is due back to the doctor on 8/11/2021 to follow up.

An Employee was walking down the stairs when they slipped on the last step and struck their arm on the midrail.

Struck by falling object

A contractor was completing demolition in the decommissioned bagging area. As the employee was pulling on an electrical local disconnect box to remove, the box fell, landing on the employee's lower leg. The employee sustained fractures to both bones in lower leg and surgery was required to repair one of the fractures.

EE was removing automatic door hinge when one of the arms of the hinge struck ee in the mouth resulting in a laceration that required seven stitches.

Over-exertion in wielding or throwing objects

EE was using a sledge hammer to loosen plug chute and reported sore shoulder to Supervisor. EE did not want to seek professional medical treatment at the time of the incident. On 7/5/2016 EE notified Supervisor that EE wanted see MD.

Fall to the walkway or working surface

EE was walking on a wet concrete slope, lost footing, slipped & fell striking EE's rt wrist.

EE was assisting in removal of the guard on the W16X conveyor tail pulley. He was positioned on the end of the guard attempting to loosen the guard from the excess material around the inside of the tail pulley. As he and another EE was trying to free the guard the injured EE pinky finger on his left hand was pinched between the corner of the guard and the wall.

EE was cleaning debris and rocks from the lift cylinder and a large rock fell on right foot.

Employee was replacing a hydraulic cylinder when the cylinder slipped and fell pinning his middle finger on his left hand between cylinder and metal step.

Miner had finished cleaning up a wet concrete floor with the skid steer when he stepped (already out of equipment) on concrete floor and slipped and fell.

Employee was completing a welding/grinding operation when he noticed an irritation to his right eye.

Struck by flying object

A choke-up of a blow line caused the system to shut down. The remedy was to remove the lid from the compressor body and remove any restricting material. The lid blew off when all the nuts were removed. The lid struck the team member on the shoulder and caused the arm to be dislocated. A small cut on the right hand also resulted.

Team member and accompanying member were preparing to place a sprocket on a shaft. The keyway needed to be dressed so that the sprocket could be installed. A hand grinder was used and it slipped from the shaft and lacerated team member's finger.

Team member was cutting approx. 8 inches off a 4 foot piece of 8 inch pipe. the balance of the 4 foot piece fell from the pipe stand and landed on the team member's right foot.

Ee and other maintenance members were repairing the ramp from the hatch opening station. The walkway grating was being lowered into the walkway framework when it fell onto the team member's left hand little finger. The damage was only to little finger.

THE INNER CAGE MILL HAMMERS AND CAGE WERE BEING ASSEMBLED. SMALL WOODEN BLOCKS WERE BEING PLACED UNDER EACH BOLT TO SUPPORT IT IN THE RECESSED POSITION. AS THE BAR WAS BEING USED TO RAISE THE RING, PRY BAR SLIPPED AND THE RING DROPPED APPROX 2 INCHES MASHING THE TIP OF THE 4TH FINGER. FINGER TIP BONE WAS FRACTURED.

TEAM MEMBERS WERE TO HAUL MATERIAL FROM SI6 TO DUMP AREA. THE TAIL GATE FAILED TO LATCH SO TEAM MEMBER TOOK THE TAILGATE AND PREPARED TO SLAM IT SHUT. HIS FINGER WAS CAUGHT IN THE LATCH ASSEMB LY.
