Fall onto or against objects

Loadout operator was starting the process of opening lids of a OTR truck at that time EE twisted ankle and fell.

Over-exertion in lifting objects

EE and a co-worker came to my office to report an injury that happened. EE stated that EE woke up this morning with back hurting, take ibuprofen 3x/day as needed. Apply ice pack 2-3x/day as instructed. EE stated EE was shoveling limestone dust off the top of Silo #4 for 1.5 to 2 hours, when EE felt a sharp pain in EE's mid back. EE said it was hard to breathe because of the pain.

Employee was climbing a ladder when EE felt a pop in the right wrist. EE experienced no pain until the following day.

Was at scales loading a truck. Loaded truck and began to return to control room Walked across scale and started to walk down a hill (about 1' high) the ground was slick. EE stepped down with right foot and it slipped from under them and they fell back on their buttocks Their right knee started to have a burning sensation

Employee was coming down the stairs (jumped) at the dryer platform and when EE reached ground level, EE's ankle rolled and EE fell to the ground, injuring EE's ankle and EE's elbow. Employee was terminated for cause on 07/22/2022 (unrelated to the injury).

Contact with heat

Employee had been working up on top of the #6 elevator for a few hours prior to lunch. After coming down from the silo, they ate lunch. After lunch, they began experiencing nausea and dizziness and was taken to the urgent care where they were diagnosed with heat exhaustion.

Struck by flying object

A contractor maintainer was using a 2-pound hammer. An approximately 1.3 cm wedge shaped shard of the hammer head separated causing projectiles that struck the maintainer through the face shield into the lower lip and chipping a tooth. A second shard struck the contractors right hand causing a laceration. The lip required sutures.

Employee while operating loader asked for medical assistance and 911 was called. Employee was removed from loader short of breath and in distress. Employee became unresponsive during first aid assistance and was provided CPR and AED while awaiting arrival of external emergency responders. Reported to MSHA immediately without knowing personal medical yet.

Fall from machine

Employee connected the hook of the SRL to a positioning D-Ring on EE's harness the hook was not properly seated on the D-Ring, but rather the D-Ring was pinched between the tip of the hook and the hook gate (not seated in the hook throat).

Employee was getting a piece of metal from metal rack when the metal fell on EE's right foot.

While repairing screw conveyor employee followed company lockout/tagout procedures but failed to realize that the screw shaft was free wheeling inside of collar when he stuck his finger into the bolt hole of the screw drive shaft.

EE WAS WORKING ON REPLACING THE WEAR LINING IN THE CRUSHER FEEDER. HE BECAME OVERHEATED.

EE WAS USING THE BOBCAT TO REPLACE SECO FEEDER BELT. IT WAS RAINING AND UPON EXITING THE BOBCAT HE STEPPED ONTO THE BUCKET AND HIS FOOT SLIPPED CAUSING HIM TO FALL ON HIS LEFT SIDE INJURING HI S KNEE, HIP AND SHOULDER.

EMPLOYEE WAS MOVING A 55 GALLON DRUM OF OIL FROMA PALLET ON A FORKLIFT INTO THE OIL SHED. DRUM TIPPED SLIGHTLY AND EMPLOYEE REACTED TO PREVENT DRUM FROM TIPPING OVER. THIS ACTION CAUSED HIM TO SCRAPE HIS RIGHT ARM AGAINST THE DOOR FRAME, RESULTING IN A LACERATION ABOUT 2.5"" LONG.

EMPLOYEE WAS DESCENDING A VERTICAL LADDER BETWEEN CONTROL ROOM LEVELS; HIS FOOT SLIPPED FROM THE3RD RUNG FROM THE BOTTOM CAUSING HIM TO LAND WITH FORCE ON HIS RIGHT LEG.

EE WAS CLOSING BOTTOM DOOR ON A RAIL CAR; THE DOOR STUCK. EE PULLED EXTRA HARD ON A 5 FT. PRY BAR HE WAS USING. THIS RESULTED IN HIM BEING IN AN AWKWARD POSITITON CAUSING HIM TO TWIST HIS RIGH T ANKLE.
