Employee was lowering door to bottom of lime pit (about 15') using an attached rope. The employees thumb got caught in the rope under the weight of the door cutting off the tip of the thumb below the nail.

Hopper chute was plugged with wet material. Process operator opened door in chute that was positioned too high to unplug with prybar. Used heavier an longer bar to unplug chute and heard a pop in EE's shoulder. Immediate pain and numbness in EE's hands and reported to Lead person. Was taken to ER by Manager and then to our doctor same day in afternoon

Employee was using a hammer to loosen the inner drill steel on the drilling equipment when the hammer slipped out of their hand and struck employee on the left upper forearm. After being fully released 3 times, On 11/5/2021 employee is scheduled for Physical therapy. At this time appointments are not made yet.

IE was unplugging a material transfer chute by banging on it with a pry bar. Felt a pinch in right hand. Went to ER on 8/27 and received a negative x-ray. Followed up at ER on 9/2 when pain and swelling persisted and received restrictions and a referral to Ortho.

EE was raking kiln 2 cooler grates when in pulling the rake back, the handle struck a hand rail behind EE causing a left shoulder rotator cuff strain. EE reported the discomfort and was sent to company doctor. EE was diagnosed on 02/06/20 and physical therapy recommended.

Employee was hammering a G-bolt out of a screw conveyor when employee felt a pop in right elbow. Employee was examined by an orthopedic specialist who diagnosed the injury as a strain. Worker returned to work with no restrictions. Follow-up visit to orthopedic specialist on 11/29/2018 resulted in doctor writing script for physical therapy making injury reportable.

Employee was changing the light assembly on the loader when EE's drive ratchet slipped striking EE in the mouth. Employee's lip was bruised and one tooth was chipped.
Employee was seated in his truck at the mine site. He was using a pocket knife to remove a new flashlight from it's package. He was cutting the package on his right thigh when the knife cut through the package and penetrated his skin.

Kiln 0perator was removing some built up scale from the kiln grates when a piece of scale contacted the rake. The rake bounced up and bumped his right ear, causing a laceration to the ear. No restriction, no impairment the doctor closed the wound with sutures and treated for infection.

Maintenance Man was removing a cut bolt from the counterweight of a conveyor belt take up pulley with a punch. When the section of bolt came out, the couterweight relaxed, causing the punch to be deflected into the mechanics finger. Caused a fracture to the middle finger, left hand.

Miner was cleaning out a chute with a hammer when right hand index (3rd) finger came in contact with some metal support structure. Causing a fracture of his finger.
Miner was patching a conveyor belt. Using a pry bar to pull the belt back together. The bar slipped off and came in contact with the right side of his lip.Laceration to the right side of the upper lip.

Ee set up a come-a-long to pull out a slide gate, and applied slight tension to hold while he went to get help to finish. The gate suddenly popped out resulting in a flow of product and cloud of dust.
