Employee tripped on stair step reporting to work striking elbow on handrail while attempting to catch balance. Elbow extension required doctor to render a shot of cortizone and prescribe physical therapy.
An gate between the lime kiln and the stone feed weigh belt was not closed and the stone feed weigh belt was exposed to hot kiln gas and caught on fire. Attempts were made to extinguish the fire but due to winds, the fire continued to smolder and required more lenghtly fire suppression as personnel could not disturb embers behind the conveyor belt guarding until cooled.
An employee was assisting with servicing operations of heavy equipment. While dismounting the lube/fuel truck, on the passenger side, the employee stepped into a rut directly below the truck's running board causing his ankle to twist. The employee was taken to a medical facility and treated for a sprained ankle.

While driving down a haul road, the haul truck struck a soft spot causing the operator's seat to bottom out. When the seat bottomed out, it caused a lower back and neck strain to the operator. The soft spot was removed and filled in with dry material.

WHEN HE TURNED TO LOOK IN THEIR DIRECTION, THE TRUCK DRIFTED TO THE SIDE OF THE ROAD, WHERE A HEAVY FORKLIFT BLADE WAS STORED. THE OFFICER TRIED TO STOP, BUT PRESSED ACCELLERATOR INSTEAD OF BRAKE PEDAL. THE TRUCK STRUCK THE BLADE, PITCHING DRIVER FORWARD.

EE WAS MARKING DRILL HOLE LOCATIONS IN PREPARATION FOR DRILLING. HE SLIPPED ON A ROCK CAUSING HIS RIGHT KNEE TO HIT THE GROUND SCRAPING IT & EXTENDING HIS LEFT LEG(IN A STRAIGHT POSITION).
EMPLOYEE WAS CHANGING A RETURN IDLER ON #2 BELT WHILE STANDING ON AN INCLINED BANK UNDER THE CONVEYOR FRAME. HE HUNG HIS 2# HAMMER BY THE HEAD ON THE CONV. BELT. HIS FEET SLIPPED ON THE BANK A ND HE PROBABLY HIT HIS HEAD ON THE HAMMER HANDLE WHEN HE FELL. THE HAMMER HIT THE BACK OF HIS RIGHT HAND BRUISING IT.
EMPLOYEE HAD PARKED PICKUP ON THE RAILROAD TRACKS AND WAS CLEANING OUT THE BED. HE THREW OUT THE MAT (ONTO THE RAILROAD TRACKS) AND SWEPT OUT THE BED THEN WHEN HE STEPPED OUT OF THE TRUCK BED ONTO THE MAT HIS FOOT LANDED ON THE RAILROAD TRACK TWISTING HIS LEFT ANKLE.

EE STEPPED OUT OF WATER TRUCK AND HIS FOOT SLIPPED OFF THE RUNNING BOARD. HE GRABBED A BAR TO KEEP FROM FALLING HURTING HIS RIGHT SHOULDER.
EMPLOYEE WAS LOADING 40-LB. BAGS OF GYPSUM INTO WAIST-HIGH CHUTE. BAG FELL INTO CHUTE AND WHEN HE TRIED TO PULL IT LOOSE, IT WAS STUCK; HE INDICATED THAT HE GOT FRUSTRATED & PULLED HARDER ON THE BAG; WHEN IT LET GO, HE FELT HIS LEFT SHOULDER POP.
EE WAS SHOVELING SPILL INTO OPENED DOOR OF BIN WHEN HI FOOT SLIPPED AND HE FELL PART WAY INTO BIN AND AGAINST THE BIN DOOR WHICH SCRAPED HIS LEFT SIDE AND HIT HIS LEFT MIDDLE FINGER.
