The employee sustained a left forearm laceration while using a pair of scissors as a draw knife to cut scrap cardboard into smaller pieces for disposal. The employee was pulling the scissors toward themselves when the scissors slipped and cut into their forearm, 4" above the wrist and glove, causing a laceration requiring 4 stitches.

While standing at an elevated position, employee lost balance when a tight bolt on the air filter housing assembly loosened abruptly. Employee fell backwards approximately 2 feet onto the concrete floor, bracing self with left hand resulting in a fracture to EE's left wrist.

Employee was holding a lug in the left hand and cutting it with a porta band saw in the right hand. The porta band saw slipped and lacerated the left index finger and slightly cut the long finger. Employee received six sutures and was returned to work full duty, no restrictions, no prescriptions.

Employee was installing shaft and bearing housing. Mounting bolt was not aligned. Employee used a pry bar to lift the housing in order to reposition the bolt. The employee reached under the housing and the pry bar slipped. Fourth finger was pinched between housing and mounting surface fracturing tip of finger and requiring stitches.

An employee was working on a Hammer Mill performing a rebuild. While EE was tightening a bolt, the wrench on the right slipped off the bolt resulting in EE's right hand moving upwards and contacting the sharp edge of the liner that was adjacent to the access door. The employee received a laceration on the top of EE's right hand.

At 3:45PM ,Saturday, Sept. 21, 2019, the operator was attempting to remove a slide gate from a chute with a pry bar. The pry bar slipped and struck them producing a laceration about 2" in length above right eye. Closure of the wound took 9 stitches at a local ER. The employee was released to work without restrictions and worked all of the following shifts.

Miner was using a sledge hammer to separate a bucket tooth from a shank which had broken off the bucket. On the fourth swing, a small shard of metal broke off the bucket tooth and struck the miner in the left leg below the knee. The metal embedded into the shin.

Miner used a drift punch and chisel to remove the top of a bearing block so the bearing could be replaced. A small metal "splinter" from the block penetrated the miners glove and embedded in the 5th finger of the left hand.

Employee was opening the bottom discharge gate on a Rail Car. He was using a 48 inch bar to turn the gate mechanism. The gear on the mechanism disengaged from the gear track as he was pushing on the bar. His momentum caused him to fall forward. He caught himself with his hands and suffered a strain in his lower back.

Employee was attempting to open a gate on the bottom of the rail car. The gate was stuck from residual materials from external sources, in addition the whether was below the freezing mark. The EE attempted to put more than normal force on the bar that opens the gate, and when the gate opened the employee lunged forward and to the left injuring causing pain in his left shoulder.

Employee was using a hand bender to straighten out a ground rod. His right, ring finger became stuck in the frame work of the bender. As he put pressure on the board, the momentum of his body caused the bender to twist. This forced his finger to twist in a sideways direction, ultimately, resulting in a finger fracture.

The gate suddenly broke free and the employee caught his finger between the bar he was using and a railroad tie. The incident was treated as a first aid. The employee sought medical attention on 10/31/12 and was diagnosed with a fracture on the tip of the finger.

The employee was unloading a rail car at the unloading screw. He was opening a rail car discharge slide using a 60" bar. The slide was difficult to open and the employee stood on the bar to attempt to move the slide. The bar slipped while he was standing on it causing him to fall to the ground. The employee twisted his right knee as he fell.

Employee was releasing a torque tool that was used to close a gate door on the bottom of a railcar. Pressure was applied to the tool handle and the release button was pressed. The handle slipped from the employee's grip, spinned around and struck the back of employee's right hand fracturing the 5th metacarpal.

The seal around the sand cooler cleanout access door was being replaced. Excess gasket material was being trimmed from around the outer edges. A utility knife was being used to trim the hard gasket material. Employee was cutting the gasket material using his right hand when the knife slipped off the gasket and made a 3" cut on the underside of his left forearm, requiring stitches.

EE felt pain in lower back when he stood up.EE received First Aid treatment on the day of injury. Employee was using a pipe to unplug the silo when he jammed the palm of his right hand on the pipe, causing a contusion which eventually became infected and swollen. Approximately seven days after the accident, the employee punctured the swelling to relieve the pressure and the puncture became infected. Therapy for wound care was done: debridement of the skin.
