Over-exertion (Not Elsewhere Classified)

Employee drilling a hole in a metal plate. Employee was fabricating a bottle rack for holding cylinders in the maint. truck. Employee was using a 1/2"" electric drill. The drill bit locked up, causing the drill to start turning. Employee was holding the handle, but the drill ""got away"" and twisted right hand, resulting in a sprain.

Over-exertion in lifting objects

Employee completed the task of replacing drive belts on the #1 attrition cell. Employee, along with the Mill Operator and the Mill Helper were installing the belt guard when the employee felt a pull in lower back.

EE missed a step and felt a pop and twist in his knee when he landed. EE refused to seek medical treatment at the time. I received documentation of EE's treatment and restrictions on 12/1, which, at that time, made it reportable. Upon further investigation, I will determine if EE's absence during this time is a result of his injury.

Fall from ladders

Employee was descending a fixed ladder when he slipped and fell. Fall resulted in broken bones (heel, rib, scapula, vertebrea). Impairment level is unknown.

Struck against stationary object

Employee was using a pipe to unplug the silo when he jammed the palm of his right hand on the pipe, causing a contusion which eventually became infected and swollen. Approximately seven days after the accident, the employee punctured the swelling to relieve the pressure and the puncture became infected. Therapy for wound care was done: debridement of the skin.

Over-exertion in lifting objects

Employee was preparing to wash down the scale aisle. He bent over to pick up water hose and got a sharp pain down the right side of his back.

EE WAS REMOVING 2X2 FT DOOR FROM NORTH DUST HOPPER ON GMD DUST COLLECTOR AT CALCINE BUILDING. WHEN LAST BOLT WAS REMOVED DUST POURED OUT DOOR & PUSHED DOOR ASIDE. EE TURNED TO MOVE OUT OF THE WAY AND HIS FOOT CAUGHT AGAINST THE PLATFORM TOEPLATE, TWISTING HIS KNEE.

EE WAS TRYING TO UNPLUG A PIPE ON THE HUMMER SCREEN DRAIN WHEN HIS RIGHT FOOT SLIPPED OFF THE PIPE. AS HE TRIED TO CATCH HIMSELF, HE BENT TOO FAR ON HIS KNEE.

WHILE UNPLUGGING A PIPE, HE IMPROPERLY LIFTED IT AND FELT PAIN IN RIGHT GROIN AREA RESULTING IN HERNIA.

EE WAS STANDING ON THE INCLINDED GRATING ON THE SOUTH SIDE OF THE TRASH SCREEN AND WAS PUSHING BUILDUP FROM THE SCREEN DOWN TO THE CHUTE WITH ASHOVEL. AS HE PUSHED HE LOST HIS BALANCE & FELL FORWARD & SIDEWAYS DOWN THE SLOPE. HE LANDED ON HIS SIDE ON AN INCLINED SECTION OF GRATING WITH HIS HEAD DOWNHILL, SPRAINING & BRUISING HIS RIBS& BACK.

EE AND ANOTHER EE WERE CHANGING OUT ATTRITION CELL IMPELLERS. THIS REQUIRES LIFTING, PULLING, AND TUGGING. LATER IN THE DAY, HIS BACK BECAME SORE.

EMPLOYEE WAS TRYING TO PLUG UP A WATER PUMP TO DEWATER AN AREA, WHEN HE STEPPED IN A HOLE. HE TRIED TO JERK HIS FOOT OUT OF THE HOLE AND IN DOING THIS, HE FELT A BURNING SENSATION IN HIS LOWER BACK.

EE WAS WORKING IN THE SCALE AISLE CHANGING BUCKETS ON E-1 ELEVATOR. DUST AND CLAY HAD GOTTEN IN HIS EYES CAUSING BOTH EYES TO SWELL (RIGHT ONE MORE SO THAN THE LEFT).
