A millwright employee was installing the bracket for the pneumatic gate air actuator for Silo 1 bin bottom while using a hammer to knock the bracket into place. As the hammer was in swinging motion it hit the employee's left thumb as it was in a downward motion to knock the bracket into place.

EE was using metal machine when material became stuck, EE tried to loosen the material by using a hammer. EE struck EE's lt thumb with the hammer causing a contusion/laceration. EE was wearing gloves. EE received 4 sutures, antibiotics and is able to RTW same day.
The employee was tightening bolts on a gearbox on the roller mill and in the process felt a pull sensation in his left inside forearm.
Lifting corner of bridge crane with 50 ton hydraulic cylinder (port-a-power) to remove cribbing from under crane. As the crane was being lifted, the cylinder was ejected from under the crane and hit employee in chest. Contusion to chest, heart, lungs, spleen, laceration to liver and broken rib(s).

THE EE WAS USING A CHAIN WRENCH TO HOLD THE #3 FAN FROM ROTATING, WHILE A SHEAVE ALIGNMENT WAS PERFORMED AND HEAT SLINGER INSTALLED. WHILE REMOVING THE WRENCH THE BACK FLOW PRESSURE FROM THE C OOLER CAUSED THE FAN TO ROTATE AND THE WRENCH PINCHED THE EE'S FINGER BETWEEN THE WRENCH HANDLE AND FAN BASE.
THE EE WAS HELPING TWO OTHER EE WHO WERE COME A LONGING A LOADING SCOOP INTO A LOADER BUCKET. THE EE SAID THE SCOPE GOT CAUGHT ON THE BUCKET LIP. HE TRIED TO FREE THE SCOPE. WHEN HE DID, THE S COPE, WHICH STILL HAD TENSION ON IT, POPPED LOOSE AND CAUGHT HIS LEFT HAND POINTER FINGER BETWEEN THE BUCKET AND SCOPE.
THE EE WAS USING A 6 LB SLEDGE HAMMER TO POUND A COUPLING ON THE COMPRESSOR TO LOOSEN IT. THE EE MISSED THE BRASS SHAFT AND HIT THE SECOND KNUCKLE OF HIS MIDDLE FINGER ON THE LEFT HAND, CAUSING A CUT.
THE EMPLOYEE WAS CUTTING BELTING WITH A UTILITY KNIFE, WHEN THE UTILITY KNIFE SLIPPED CAUSING A LACERATION TO THE EMPLOYEE'S ABDOMEN.
THE EE HAD POSITIONED HIS FEET ON THE GRIZZLY BARS AND THE LOWER LIP OF THE COOLER DOOR FRAME, AND HIS BODY WAS UP AGAINST THE SOUTH WALL, IN ORDER TO USE A SLEDGE HAMMER TO BEND A PIECE OF ST EEL. WHENT HE EE SWUNG THE SLEDGE HAMMER TO STRIKE THE PIECE OF STEEL, THE SLEDGE HAMMER MISSED THE TARGET AND HIT THE INSTEP OF THE EE'S RIGHT FOOT.
THE EE WAS INSERTING PINS IN THE PREBLEND FEEDER. THE EE HAD INSERTED THE PINS BY HAND, AND WHEN FELLOW EE WAS SWINGING THE 8# SLEDGEHAMMER TO DRIVE THE PINS IN THE HEAD CAME OFF THE HANDLE OF THE HAMMER, STRIKING THE EE'S LEFT THUMB.
THE EMPLOYEE WAS WORKING ONT EH BURNER PIPE, WHE HE TAPPED ON THE BURNER PIPE, DUST FELL INTO THE EMPLOYEE'S LEFT EYE.
THE EMPLOYEE WAS USING A SLEDGE HAMMER TO LOOSEN THE DOOR ON THE FINISH MILL. WHEN USING THE SLEDGE HAMMER THE EMPLOYEE ALLEGEDLY FELT A SHAR PAIN IN HIS BACK.
THE EMPLOYEE WAS OPENING A GATE VALVE WHILE MAKING A BIN CHANGE, AT WHICH TIME THE BAR THE EMPLOYEE WAS USINGSLIPPED OFF THE NUT AND STRUCK HIS LEFT THUMB, CAUSING A FRACTURE AT THE TIP OF HTE LEFT THUMB.
