Employee received a piece of metal, embedded in his upper thigh area, from removing teeth from a loader.
Employee was opening a box with a box cutter. As the employee turned the box around, on the third turn, he accidently lost control of knife as it came towards his left arm. The knife lacerated the left wrist and at the same time puncturing an artery.

The Counterweight did not line up with the saddle, a come-along was attached to the frame on the conveyor. He took a closer look to see what it was hanging up on and in the process put his hand on the frame of the take-up system, the weight shifted catching the tip of the middle and ring finger on his right hand between the frame and the weight.
Striking the hammer against the chisel cause a small piece of the hammer face to came off and went into index finger on left hand.
He was attempting to dislodge a rock in the winged tail pulley with a long piece of flat bar. He pulled down on the bar, the end of the bar rolled out of the tail area, the flat bar smashed his left index (pointing) finger resulting in a broken bone, 5 stitches. He was wearing gloves on his hands at the time.
Employee was removing driveshaft from 972 loader. Employee was striking driveshaft T with hammer to remove from yoke. At the area of impact on the bearing cap a piece of metal broke free from the cap and punctured right calf of employee. Size of metal was approximately eigth of an inch in diameter.
Employee was splicing the wire for the installation of a replacement light fixture on the #2 Hazemag crusher, as he was cutting off the outer cover of the cable the knife slipped and cut his thumb.
Employee was working on a feed throat for a FM. While trying to clear the plug with a sledge hammer and rod, employee smashed his hand causing laceration and fracture to right middle finger.
Employee cut right index finger while removing zip tie.

Moving a cable tree when the chain used to move the tree slipped and pinched the employee's finger.
Employee was clearing a plug on the FK Pump for #3 finish mill. He had cleaned out the pump and screw and was working on the flapper when cement that was in the line to the silo came back down the pipe and hit him. It knocked off his hat and glasses and got into both eyes. He wasn't wearing goggles as required.
Employee was removing plastic zip ties from computer cables and in doing so cut left wrist with knife.

EE WAS ADJUSTING THE FEED END AIR SEAL ON KILN USING A PORTAPOWER. THE SEAL SHIFTED QUICKLY AND DISLODGED SOME KILN DUST THAT ENTERED THROUGH THE GOGGLES AND INTO HIS EYES.
EE WAS MOVING HAMMERS IN THE STEDMAN WITH A PRY BAR WHEN THE HAMMER JUMPED OVER THE RAIL. EE GRABBED THE HAMMER TO KEEP IT FROM FALLING OFF THE TABLE AND STRAINED HIS BACK.
EE WAS HELPING INSTALL A PIPE UNDER A CLASSIFIER SYSTEM WHILE USING A CROWBAR TO APPLY PRESSURE TO KEEP PIPE IN SLOT SO IT COULD BE CLAMPED, WHEN HE PULLED HIS LUMBAR MUSCLE.
WHILE UNDOING BOLTS FROM MACHINE, A PIECE OF A RUSTY METAL BOLT GOT INTO THE EMPLOYEES RIGHT EYE.
EE WAS CUTTING BAND ON A BUNDLE OF EMPTY BAGS. HE WAS PULLING THE KNIFE TOWARDS HIM. WHEN HE CUT THROUGH THE STRAP THE KNIFE CONTINUED ON AND HE STABBED HIMSELF ON THE INSIDE OF HIS LEFT THIGH .
EE WAS CUTTING BUT HAD HIS LEFT HAND DIRECTLY ACROSS FROM HIM. THE CUTTING UTENSIL SLIPPED AND HE CUT HIS LEFT THUMB. (THE DELAY IN REPORTING THIS WAS NOTED DURING AN INTERAL AUDIT).
WHILE ATTEMPTING TO KNOCK A TOOTH OUT OF A LOADER BUCKET A METAL FRAGMENT PUNCTURED HIS RIGHT ARM.

A FUEL/LUBE TRUCK DRIVER WAS CHANGING THE TEETH ON A 990 LOADER, USING A HAMMER 3 LB WHEN UPON STRIKING THE PIN A METAL PIECE HIT HIS UPPER LEFT LEG.
EE WAS USING A CHISEL BRAND NEW TO SPREAD LINKS ON A TRACK SYSTEM. THE CHISEL SNAPPED IN HALF AND CUT HIM ABOVE THE RIGHT EYEBROW.
EMPLOYEE WAS SWINGING A SLEDGE HAMMER WHILE DOING MAINTENANCE IN THE PLANT AND STRAINED HIS RIB CAGE.
EE WAS WORKING ON A CRUSHER. HE GOT A PRYBAR STUCK TRYING TO REMOVE A CHIPPING HAMMER. HE COULD NOT GET HAMMER OUT SO HE THREW PRYBAR DOWN ON GROUND. WHEN HE THROW PRYBAR DOWN A PIECE OF METAL WENT INTO TO HIS LEFT EYE. NO SAFETY GLASSES BEING WORN AT TIME.
