Operator inspected the mill tank for plugage. During inspection EE found a plugged area around the discharge of the tank. EE then retrieved a cleaning tool from the milling area and began prying product away from the discharge area. While clearing the product away from the discharge, the tool slipped from EE's left hand and struck EE's right index finger

Showing all 12Employee was cleaning out the #5 Mill feed bin with a push bar to clean the material that had plugged up inside the tank. When the employee pulled to remove the push bar employee's left index finger became caught between the push bar and a guard rail.
Two employees were using a metal bar and forklift to free a compacted bucket elevator. The injured employee was in the line of fire when a bucket broke over causing the bar to jolt upward. Injured employee was struck by the metal bar causing a laceration requiring stitches on the left hand palm side thumb at the second knuckle.
The accident occurred on 9/25/19, but it didn't become reportable until EE went to doctor on 10/03/19. EE first noticed pain while pulling wrenches on the Haver bag placer, one in each hand, to either loosen or tighten. Then moved to hammer to help move a bolt. Moved back to wrenches and pain got worse. Pain present along forearm to palm near thumb on right hand.
Miner was performing a gear change out on the Raymond Mill inclined grit auger. Miner was using a small sledge hammer to assist removal of gears when their hand struck an adjacent guard constructed from expanded metal. The miner received a laceration on the back of their hand that required sutures.
Employee was cleaning out hard built up clay/mud out of Imp Mill Slicer. While employee was trying to remove clay/mud from the back walls using a heavy bar in a prying motion. The bar slipped causing a quick and hard motion resulting in a sharp pain and burning sensation in the left forearm.
Changing hammers in hammer mill when left hand index finger was hit with pry bar
Using a pry bar to hold a part in position and the bar slipped striking employee in the front tooth, breaking his tooth.
Employee was cutting a piece of PVC well casing from around a well pipe with a hand saw, and the saw slipped from the groove and cut his knuckle. Saw lost contact with pipe.
The employee was performing service work on a dozer while standing on the dozers tracks at 36" from the ground. The employee was trying to break a bolt loose when the wrench slipped causing him to fall to the ground. When tried to catch himself when hit the ground he broke his right wrist.
While racking loose material from underneath a rail car employee developed stiffness in his back.
EE WAS LETTING RAIL CARS DOWN AT THE DRYER. WHEN HE STARTED TO TIGHTEN THE BRAKES ON ONE OF THE CARS HE FELT A SHARP PAIN IN HIS LOWER ABDOMEN. HE STRAINED HIS LOWER LEFT ABDOMEN.
