Employee reported pain in L hand/wrist consistent with carpel tunnel. Employee evaluated by orthopedist and surgery recommended.

Employee was cutting a piece of PVC well casing from around a well pipe with a hand saw, and the saw slipped from the groove and cut his knuckle. Saw lost contact with pipe.

Flash burns (electric)

Employee went to the well to get a water sample. Tried to start the pump by turning on the breaker. He opened the door to the breaker and saw the breaker was tripped, so he tried to jump the breaker using a jumper

EMPLOYEE GOT OUT OF VEHICLE AND SLIPPED ON ICE, TWISTING HIS LEFT ANKLE, CAUSING A HAIRLINE FRACTURE.

EE was assisting with moving and loading steel rods. One of the rods rolled and slipped out of his grip, finger got caught between two rods, mashing middle finger.

While removing plates in the 30"" x 42"" Austin Westin jaw crusher, one welded eye disconnected as the crane was lifting the plate. The welded eye struck the injured employee in the upper left chest area breaking #6 and #7 ribs.

Over-exertion (Not Elsewhere Classified)

EE was tightening bolts in jaw crusher whe he felt something pop in his left shoulder. It was determined that the trapezius muscle in left shoulder was pulled. As of 3-30-04, ee has returned to work on light duty.
