Removing a guard on the push arm of a 375A bulldozer to make a minor repair using a wrench. Wrench slipped and struck employee on right shin causing a laceration.
EE was using a utility knife to cut bucking rubber. As EE was shaving the bucking rubber which was on a steel sheet, the sheet moved and began to fall over. As EE went to grab the steel EE unintentionally cut hand with the utility knife. EE received 5 stitches on the inside of palm near the thumb.
HELPING CHANGE THE HOIST ROPE ON THE 7450 DRAGLINE. WAS REMOVING THE WEDGE FROM THE HOIST CABLE SOCKET USING A RUBBER SLEDGE HAMMER WHEN EMPLOYEE INJURED ARM WHILE SWINGING.
Employee was cutting a piece of channel rubber to length with a utility knife. EE was kneeling on ground and cutting rubber on a pallet in front of them. They cut the rubber towards themselves, and with such force, that after making the cut, the knife continued on and sliced thru their coveralls and work pants. EE sustained a laceration on the right thigh - 7 stitches.
Cut finger while using a knife to cut a piece of belt.
Employee was using a sledge hammer to hit a tooth on the 992 bucket to remove the tooth. A small piece of metal broke off the tooth & hit employee in the thigh.
Tightening bolt on side seal area of bagging machine when hand slipped hitting the corner of a piece of metal on the machine.

Employee was stripping a wire for the turn signal on truck #83 at the truck maintenance garage. He was using a razor blade. The blade slipped and cut a finger on his left hand.
Beltrami was tightening a spring break on #85 tractor with a wrench and pulled his right shoulder.
Climbed on top of 777 truck to put tow cables on in order to pull truck. This resulted in pulling or pinching testicles.
Employee was installing a bearing on a 4600 dragline. We used a chisel to wack it on, shattering the end of the chisel and resulting in a 5/16 inch piece of steel lodging into his stomach (right lower abdomen)

Was under loader and got dirt/metal in eyes.
EE was using a pry bar to put pin in track of Demag excavator, when another person was hitting the pin with a sledgehammer and a piece of pin broke off and hit employee in the upper lip. Note: EE did not seek treatment until 1/17/2013.
Employee was removing cylinder pin on Demag bucket using a metal bar. When pin was pushed through, cylinder slid against bar, causing hand to be pinched between bar & bucket.
Hit in head with hammer
Heating and bending round stock with sledge hammer. Woke up next morning with what he thought was tendonitis. Went for x-ray on 5/31/11 and found out hand was fractured.
EE was using a ratchet and pipe to loosen bolt. Ratchet broke and hit his finger between the pipe and ground.

As another mechanic was using a hammer to drive a steel pin into a house roller of a dragline shovel, a piece of metal slag broke loose while hammering and struck EE in the corner of his right eye, while standing at least 10' away.
WRENCH SLIPPED AND LEFT HAND HIT OBJECT AND BRUISED. INITIALLY IT WAS THOUGHT THAT THERE WAS A FRACTURE OF THE FINGER. TEST AND X RAYS WERE DONE. SEVERELY BRUISED.
