A 3/4 inch air gun twisted out of hand and pinched hand between the dragline and air gun, causing left wrist to twist. Employee did not seek treatment or have lost time until 1/6/23.

Employee was laying on EE's back in an awkward position using a pry bar trying to pry a pin out of the bore on revolving frame on the 4600. Doing so EE strained EE's back.
Employee noticed dozer was not operating well and pulled out of dump area and requested someone to look at dozer. Then the engine caught fire and employee jumped approximately 8 feet to the ground landing on ankle and shoulder causing a strain/muscle tear on right shoulder.

EE was using a utility knife to cut bucking rubber. As EE was shaving the bucking rubber which was on a steel sheet, the sheet moved and began to fall over. As EE went to grab the steel EE unintentionally cut hand with the utility knife. EE received 5 stitches on the inside of palm near the thumb.
Continuing pain from running the haul truck all day. Caused shoulder/neck pain.

Hearing Loss - threshold shift in right ear
EE came out of the man door of the welding shop and walked in front of the garage door to walk over to the demag bucket they are working on. EE walked on some ice and slipped. EE didn't fall down but caught self and did a half split. That's when EE pulled the muscle in EE's leg.

Climbing down off the D10 to do a walk around inspection, EE missed the step. EE was holding on to the grab handle to catch self when EE fell and EE's body twisted, hurting EE's shoulder.
Injured employee and another employee were taking 4 lengths of square tubing off the rack to be cut in half. Injured employee was bending over to lower the injured employee's end to the ground and hurt back.
Employee was cutting a piece of channel rubber to length with a utility knife. EE was kneeling on ground and cutting rubber on a pallet in front of them. They cut the rubber towards themselves, and with such force, that after making the cut, the knife continued on and sliced thru their coveralls and work pants. EE sustained a laceration on the right thigh - 7 stitches.
Employee was using a sledge hammer to hit a tooth on the 992 bucket to remove the tooth. A small piece of metal broke off the tooth & hit employee in the thigh.

Employee was standing on a ladder plugging in a light at top of a bucket of a front end loader. When EE stepped down, EE missed a step and fell into a pile of steel plates (from inside the bucket), hitting head.

Employee hurt back from lifting teeth for DeMag while building up the base.
Employee was grinding rust and slag from a chute. He was using a 4"" grinder and wearing tight fitting, clear goggles. When he removed his goggles, debris from the top of the goggles and/or forehead fell into his eye.

Employee was sitting on a bench in the break room having a drink of water. Wind caused a window on the wall to swing open (window swings down). Window crashed over employee's head and cut the bridge of his nose.
Employee was walking across the parking lot to get to the welding shop. He slipped on the ice and landed on his left arm. The ice formed because snow melted the day before and refroze overnight on the parking area.

Employee was carrying and tossing scrap metal into front end loader bucket. He experienced a muscle spasm in the right side of his neck and down to his right shoulder.
Employee was installing a bearing on a 4600 dragline. We used a chisel to wack it on, shattering the end of the chisel and resulting in a 5/16 inch piece of steel lodging into his stomach (right lower abdomen)
Went to annual audio test and found a STS of 20dB at 2000Hz in Right ear. Repeat test in controlled environment showed a shift in the left ear at 3000 Hz with STS of 10 dB. They recommended he goes to audiologist who confirmed a 14dB STS in the right ear.
Grinding on dragline bucket when a piece of slag got past his safety glasses and burnt into his right eyelid.
Employee was grinding bucket bushings on Caterpillar 5130 excavator. No specific incident. After time, right eye became irritated. Sought treatment about one week later. Not reported until we found out he sought treatment as no lost time.
Welding on dozer blade laying down and other welders arc went through the back of his helmet, resulting in welding flash in both eyes.

EE was using a pry bar to put pin in track of Demag excavator, when another person was hitting the pin with a sledgehammer and a piece of pin broke off and hit employee in the upper lip. Note: EE did not seek treatment until 1/17/2013.
Pulling air hose from under equipment and lacerated left hand on sharp object.
