Ee was changing bits in the miner head. He had the bit wrench in his left hand and small sledge hammer in his right hand. He put the bit wrench on the bit close to the bottom of the head. While attempting to strike the wrench, he struck his right index finger against the bit wrench or bit block.

Showing all 8EE and other were trying to tighten a nut on the stacker belt take-up. They had a pipe wrench on the nut a cheater bar on the pipe wrench and both ot them were pushing on the cheater bar. The wrench slipped ee mashed his finger between the cheater bar and frame of stacker belt.
Employee was loosening bolts and when they loosened the ratchet let his hand strike a sharp edge cutting the right pinkie (five stitches).
EE WAS HITTING HAMMER AGAINST HAMMER TRYING TO GET TIE OFF BLOCKS OFF BELT CLAMP WHEN A PIECE OF METAL WENT INTO THIS LEFT FINGER (FIRST FINGER). HE WENT TO THE DR. ON MONDAY JUNE 28, 2004 TO GET METAL OUT OF HIS FINGER. HE WILL RETURN TO WORK ON HIS WEDNESDAY SHIFT JUNE 30 ,2004.
EE FEEDING ROCK DUST INTO THE FEEDER HOPPER OF TOF THE DUSTER. ROCK DUST STOPPED UP IN DUSTER HOPPER. EE TOOK HIS HAMMER HANDLE AND STARTED PARKING THE DUST IN THE HOPPER. HANDLE WENT THRU GUA RDING GETTING CAUGHT IN AUGER, WHICH CAUSED HIM TO CUT AND BREAK RING FINGER ON RIGHT HAND.
HE WAS DRIVING A SPAD INTO COAL RIB WHEN HE STRUCK HIS MIDDLE FINGER ON HIS LEFT HAND.
WHILE TAKING BELT BLOCKS OFF BELT, EE WAS TAKING ONE SIDE OFF, HE WAS HITTING THE BLOCKS WITH A HAMMER. THE BELT JUMPED CAUSING HIM TO HIT HIS LEFT HAND.
EE WAS HELPING MECHANIC ADJUST THRUST RODS ON BOLTER HEAD. EE PULLED ON PIPE WRENCH CAUSING HIM TO PULL SHOULDER AREA. RT. SHOULDER STARTED TO HURT.
