Employee inadvertantly cut himself while shoveling the #15 tailpiece. A utility knife was on his belt and the blade had worked itself out and he struck the blade with his left hand.

EE WAS PULLING SLACK OUT OF BELT WITH VISE GRIPS AND THEY SLIPPED AND STRUCK HIM IN THE LEFT EYE.
EE WAS USING A BELT KNIFE TO OPEN AN OIL CAN AND THE KNIFE SLIPPED CUTTING HIS LEFT HAND.
Employee alleges he twisted his right knee while shoveling the #1 belt line. (sli and fall). Employee continued to work until 5-26-04 and began missing work at that time.
