Injury was cleaning gob in 3 left with continous miner, he started backing out of crosscut when corner of rib rolled off on him receiving a fatal injury to his upper torso. Size of rib striking he was approx. 4'L x 36""W x 16"" thick.

After cutting a speed reducer loose from 5 head drive the speed reducer slid from head drive falling on employee's left arm. Due to his injury employee received a fracture to his left arm, employee has returned to his regular work duties.
While doing weekly examination a rock fall was found on 4 main Break 17 in the #4 entry. The size of the fall was 8' thick 18' wide and 30-35' long. The ventilation is not affected. Fall occurred due to a transition zone in the top. An action plan has been submitted and is still pending.

Employee was on 8 beltline at brk.21 repairing a brattice, employee was on his knee's when he reach over to get a block, a block from the brattice fell out striking him in the back fracturing 3 vertebrae's in back. Employee became a lost time due to this injury.
A roof fall has occurred at Transition Mains, at break 33 in the #1 entry intersection over to the #2 entry. Area will be crib, Heimans beams will be installed and 16' rope bolts will be installed to prevent future falls.

Employee is a scoop operator that was loading a bundle of roof bolts that were binded together with a metal strap, the strap broke loose striking employee on right forearm, cutting arm, employee received 6 sutures and returned to work with no restrictions.

Bucket of scoop caught corner of rib and broke rib loose causing rib to roll onto EE and roof bolting machine. Employee received a fractured collar bone and 6 left ribs were fractured, employee became lost time on 12-10-09.

On 7//21/09 employee was plastering a stopping when ladder fell spraining his left ankle, employee had returned to regular duty,it came to my attention recently that the employee had returned to work on 7/22/09 but not to regular duty, light duty. Employee was on restricted duty for 1 day.
