A non injury accident occurred in a form of a rock fall between #2 and #3 entry at break 15 app. 20ft by 20ft. It was 4 ft. thick and fell at the point of anchorage. Timbers were set. Brattices were built to reestablish ventilation with Heisman Jacks and beams set in front of the brattice.

Roof fall information from D-2 in the plant mains found at 7:40am. It was reported to the call center at 7:45am. The roof fall is in #4 and #5 entry beginning in x-cut 5 thru 6. In #4 entry the roof fall is approx. 70'long x 20' wide x 4-6 high (thick).
Employee had previously twisted a knee while tightening bolts but continued to work, sought medical attention on 12/18/2018 where EE underwent a procedure and received stitches to rt. knee and returned to work the next day. No time lost.
NON-INJURY roof fall occurring at crosscut 45, on the #4 belt - in the intersection and to the right and left of entry.

Employee was using a utility knife to cut tape & knife slipped and cut employee's hand. Employee received 4 stitches.
Employee stated that they were taking the scrubber motor ductwork off the miner when it mashed a finger. They received 10 stitches to their ring finger.
EE was taking a 1 1/2"" water line loose at the pump when water line sprayed EE in the face and some debris got into eye under the safety glasses. EE went to ER initially, followed up with the eye doctor. EE had piece of metal in the right eye and it was removed. EE has no lost time. EE has returned to work.
EE squatted down when raised up & turned felt pain in lower back, went on to section started hanging curtain. EE said pain got worse, went to local ER. EE stated the dr. said it was muscle spasm. EE went to family dr. next day, EE is off work do not have a return to work date.
While working on cable reel on shuttle car, lid covering reel fell and hit EE's hand. EE went to ER and has a broken little finger and received 9 stitches. EE is off work until EE sees orthopedic doctor.

Roof fall break #8 plant mains #5 entry. 20' wide X 25' long X 5' thick. Roof/rib support in affected area: shale 4 or 5' resin bolts.
Employee moved shuttle car tire out of way with help of another employee. Bent over to pick up a pogo stick and felt pain in back.
Changing out belt control box, was pulling old box out of area, employee started to back out of way as old box started to turn over on EE's right lower leg and foot. EE had a cut half way up EE's leg on EE's shin requiring 9 sutures.
Roof fall was located in belt entry #3 of #4 belt line at break #60. Fall is approx 20'x30' and approx. 7-8' high. Cribs, Steel beams, jacks, and 10' cable bolts existed in the area prior to the fall. An action plan was submitted and includes plans for cleaning up and rehabilitation of the roof fall. The fall did not affect ventilation, miners, or equipment.
There was a roof fall located at break 12 in the #4 entry of the plant mains. The approximate size of the fall was 20' x 6' x 40'. It has been dangered and cribbed off and the #5 entry will be used as the travelway around the fall.

Miner cable was grounded and employee was using fault wizard to find bad spot. He thought he had it and took the fault wizard off and began using his meter to check if cable ground was cleared. This is when employee received a shock.

Employee had placed left hand on scoop bucket after hanging up 6"" waterline when, water line fell striking fingers on left hand. Employee received 6 sutures to his 3rd digit finger and hairline fracture to 2nd digit finger on left hand. Employee has returned to work with no restrictions.
While doing a weekly examine a rock fall was found at Shell Branch Bleeder in the #2 entry at brk. 25-26. The size of the fall was approximately 4 to 5' thick 20' wide and 40' long. The ventilation has not been affected, cribs are being installed in the affected area.
A roof fall was found at 67 Mains, on 6 track at break 8 between 4 & 5 entry. The fall was approx. 2' to 3' thick 18' wide and 30' long. 10 Heintzman Jacks with pizza pan heads will be set. This fall does not block any escape way or effects the ventilation.

A contract delivery truck driver at the Mines with temperatures around 100 degrees got out of his truck the air conditoner was not working at the time unstrapped a load of pipe when he staggered to the ground suffering from heat exhaustion, went to the hospital and doctors informed him that it was not life threatening but would like to keep him to check his heart.

At 13 belt drive at a ( 500 KVA power center) there was an electrical fire in the transformer of the power center located in 67 mains, the power center was de-energized, miner's were evacuated, fire was extinguish, all local agencies were contacted, no injuries, and power center has been removed from service.
