Employee was chipping tiles out of a TEMA in the prep plant. Employee was wearing safety glasses and a full-face shield. Employee felt something in the right eye and went a mirror to find a foreign debris in the right eye. Employee asked for assistance from another employee to flush eye. The employee was taken to a local optometrist and prescribed antibiotics

Employee was pulling cutting motor on longwall with chain and come-along and the hook accidentally broke causing the hook to accidentally strike employee in the shoulder causing injury

IE stated that IE was trying to unplug a chute in the prep plant screen building using a scaling bar. IE was attempting to bust up material from the chute working upwards. The debris came loose falling below. The weight of the material sloughed down the chute catching the bar. The bar springboarded upward striking the IE on the left jaw, causing a laceration requiring 13 sutures.

IE was cutting a hole in belt to put rope through to pull belt. EE was cutting right to left. Left hand was on the belt, knife was in the right. The left hand was cut on top of the hand when right hand continued it's direction of travel after knife blade broke. IE was wearing EE's cut-proof gloves at the time of the incident. This was submitted in Oct., but it's not on the DRS.

IE was installing roll pins in the flights in the feeder. IE was driving them in with a hammer and missed striking IE's left thumb, which was holding the pin with the hammer. The blow caused the thumbnail to be removed from the cuticle and exposed. The IE received sutures.

Employees were replacing canopy on shuttle car. His left index finger was in the hole as he was evaluating the situation. Employee on the top of the canopy swung the hammer to tap that corner down into the sleeve causing the canopy post to lower into the sleeve, amputating the tip of the IE's finger from the finger nail to the tip (tissue only).

Injury occurred on 03-29 at 3AM but not reportable until 04-03. IE was laying on the ground swinging a hammer trying to knock out a brace pipe that was causing the cable reel to stick. The brace/guard was bent by a rock in road or the ruts in the road at the feeder. The pipe was moving and the IE swung again, missed the pipe and struck his shin, just below the knee.

Employee was replacing pins on the shields using a slide hammer. While using the slide hammer he swung back and struck his finger between the base of the slide and the hammer. (six stitches were required)

Employee states he was in the process of pulling the bit sleeve off a miner. The sleeve was tight on the miner, so employee used the end of a chisel to shim the ram, in order to push the sleeve out. As he removed his hand from the chisel, the chisel shot out and punctured employee's right hand.

Employee states he was removing the boomer and chain from a load of lumber. As he was prying on the boomer it released and the handle struck him on the left wrist. The left wrist required surgery on 4/28/2009.

EE was shovling on 2-main N belt and a timber was setting in glob instead of a hard surface floor. The shovling loosened the timber and fell stricking EE on the back left shoulder. Post accident drug teat read non-negative, awaiting lab results.

Employee was preparing to remove a headgate trapping shoe and had positioned a porta-power cylinder against the pin. As a co-worker started to hand-pump the power pack, a fitting connecting a hose to the cylinder failed, allowing the pressurized hose to strike the employee's forehead. He received a laceration requiring numerous stitches.

EE states as he was tightening a bolt on a drive line, he felt pain in his right shoulder. He continued to work; however, on 8/3/06 he advised he was still feeling pain and requested medical care. He continued to work until his surgery on 8/15/06 and he returned to light duty on 8/21/06.

Employee states he was using a hammer to knock the ring gear off an engine flywheel when a piece of metal flew off and hit him in the cheek. The sliver of metal was removed from his cheek and required one stitches.

EE STATES EE WAS PUTTING IN BELT HANGERS AND THE WRENCH WAS COMING OUT OF THE CHUCK. AS EE WENT TO GRAB THE WRINCH, THE STEEL CAME OUT OF THE ROOF AND EE FINGER WAS CAUGHT BETWEEN THE STEEL AND THE WRENCH. EE RECEIVED A LACERATION ON THE RIGHT INDEX FINGER THAT REQUIRED STITCHES AND EE WAS PLACED ON LIGHT DUTY FOR 2-3 WEEKS.

EE STATES HE WAS INSTALLING A PLUG ON THE END OF AN EXTENSION CORD. WHILE TIGHTENING A SCREW, THE SCREWDRIVER SLIPPED AND PUNCTURED HIS LEFT HAND. THE WOUND BECAME INFECTED AND HE WAS ON RESTR ICTED DUTY THREE DAYS.
