Showing 50 of 581A Mill Diagnostic mechanic was guiding a coupling guard into position using crane assistance. The mechanic attempted to lift the bottom edge of the guard to maneuver it over the adjustment bolt. The guard was set back down, the mechanic's left hand was still positioned underneath, resulting in the left index finger being caught and pinched between the adjustment bolt and the guard.
Employee was tailing a core tube out from a ct-20 to pull the sample. While tailing out the tube, the Kelly hose snagged on the wire line causing slack. As the tube came up changing the angle releasing the slack, dropping the back side of the tube 1 foot causing a strain on the helper right shoulder
Operator was attempting to fill in a hole from where the shovel came out of. While pushing material, the operator claims that they came to a sudden stop causing them to be jarred forward. Operator went restricted duty on 3/11/2026.
A Haul Truck was being loaded by a Shovel. During the third dipper, a boulder was at the top of the dipper leaning on the teeth. When the bottom of the dipper opened, the boulder did not fall through with the rest of the load. Instead, it rolled over the teeth, hit the driver side of the haul trucks canopy and fell to the ground. Operator went restricted duty on 3/12/2026.
The Shovel Operator walked the shovel back down off of the 3'-4' hard knob and the shovel jolted. This resulted in the operator experiencing neck pain.
A Mechanic was removing the propel shaft on a Shovel using a steel cable and a Dozer. While positioning the cable on the dozer's ripper, a mechanic placed a hand near the line. When the dozer moved forward, the cable shifted and caught their left hand between the cable and the ripper.
Two employees had checked a belt and were walking on the lower level of the screen plant when a rock from the next level up fell over toe board striking one of the employees on top/right side of their head.
While grinding the employee felt a foreign object enter their right eye. A small metal fragment was removed on-site, and the eye was flushed. The employee was transported to the local occupational clinic, where no remaining foreign body was found, but an injury to the cornea consistent with the flushed object was observed.
Replacing return roller on locked out conveyor belt system when jack failed and belt pinned hand against roller. No broken bones, pinched muscles and loss of sensation to hand.
CC Employee was welding a wear liner block to the crusher saddle liner, in the process of building material with weld, molten Material size of a dime dropped and landed on top of pants where the top of where boot opening is. The molten material burned through pant leg and entered the boot landing on the top of the left foot. Causing a burn.
CAT79 experienced a rough load event while being loaded by Shovel 44, positioned on the outer edge of the cut and working in course material.
Employee felt pain in back after hitting a hard knob
Haul truck CAT82 was in the process of being loaded by Shovel 44 when, during the initial bucket load, a concealed boulder within the material struck the truck bed, resulting in a rough loading event.
When shovel dipper was coming out of the bed of the truck, a rock came out of the dipper and struck the dovetail, jarring the driver. Driver was taken to the ER and given time off, missing next shift, fully released on 11/03/2025.
Employee was adjusting lifting shackle and overhead crane hook during lifting operations. The employee's right index finger was pinched between lifting shackle and overhead crane hook. The employee suffered partial traumatic transphalangeal amputation of right index finger with nailbed involvement.
As employee was making their rounds in the grinding tunnel, a grinding ball rolled down the tunnel, striking them in the left calf. EE was taken to the ER, diagnosed with a contusion and placed on restricted duty. CURRENTLY ONGOING.

EE was removing a 20" hugger with an impact when the nut came off of the bottom half of the hugger it dropped. The EE caught it and tweaked EE's back in the process. On 9/19/25 EE was seen and sent back to work. On 9/25/2025 EE was issued physical therapy.

Mine Maintenance Employee struck/dragged across ground approximately 6 feet by SH22 hoist ropes during maintenance activity.
Employee was checking froth quality at cleaner cell when finger became pinched between froth paddle and cell weir.

Production Superintendent was attempting to re-rig an air winder to a forklift to be picked and put into place for a belt change out. The air winder was placed on a wood core for cribbing. The air winder load shifted and contacted the employees left foot causing a fracture.

Employee was welding a beam to the structure. There was another welder from behind grinding material. Employee noticed sparks coming up from behind. Employee noticed a spark came in under the welding hood and landed under neath their safety glasses. The spark hit the employee in their eye. Employee imediatately flushed their eye and called for saftey.

Employee was removing panels from screen media with Air Tugger, when employee was holding Air Tugger chain with EE's left hand. Employees left hand became lodged in the Air Tugger Block causing laceration to EE's left hand.
As the crew was tripping 20-foot rods, the driller assistant (DA) requested an adjustment to the jib boom. Driller contacted the head tilt lever instead of the Jib boom causing the rod to move forward unexpectedly and striking the DA between the chin and the lip causing a laceration that required 4 stitches to close.
The employee was working at the CLP Plant jack hammering concrete, when the jack hammer bounced up and landed on the employee's left foot. The jack hammer tip struck the boot between the steel toe and the Metatarsal cover. Causing a fracture to the left big toe.
Employee's right index fingertip was amputated inside the pin hole of the Tube Press 6 ram when the Overhead Crane raised the yolk out of the candle eye.
While loading Haul Truck 36, a shovel operator trainee made contact with the back of HT36 with the dipper. After dumping the operator continued onto an additional run at Shovel 3. After another full load/dump cycle, the operator stated they felt neck and back pain.
IP's 4 fingers on right hand were crushed between Horse head and block & tackle of service truck mounted crane. IP required surgery on all 4 fingers with a portion of middle 2 fingers requiring amputation
EE was using an impact wrench to remove bolts from the wear segments on a loader. EE placed a hand on the bottom edge to hold the carriage bolt up while using an impact gun. It spun the nut off. The bolt head spun causing a laceration to EE's left index finger and middle finger. EE was wearing protective gloves at the time. EE received stitches and restrictions.

Employee jarred back while operating shovel
Multiple oversized boulders were loaded onto haul truck HT107, creating a rough load event. This sudden impact resulted in the operator sustaining a back injury severe enough to require a mayday call over the radio. No vehicle damage occurred; however, the operator's injury required immediate medical evaluation.
Employee got foreign debris in eye.
Employee received first bucket from shovel 2 and noted to the operator and Dispatch/Supervisor that the load was rough. They stated at the time they were OK, needed no medical attention, and incident was initially classified as first aid. On 10/24 upon follow up with Occ Med Provider, employee received restrictions.
Employee was grinding overhead, when they removed their face shield, they felt eye irritation, foreign body was discovered that required removal techniques beyond first aid.
Employee was cutting cathode samples when EE moved the table for the next cut. The employee had both hands placed on the table with EE's thumbs placed underneath while in motion. The employee's right thumb was pinched in between the wheel and the table, diagnosed with a contusion, taken off work. Currently ongoing.
EE was doing a fix on a drill. They were using the torch for steel bars. Their left hand was holding the bar & they turned to the right. The torch began to heat their glove. When they felt the heat they turned off the torch, removed their glove & saw their hand was burned. They were taken to the clinic & injury was debrided. Employee was issued restrictions.
An ee was cleaning the rust off a dog ear by placing it flat on the scaffolding floor. A coworker was climbing on the scaffolding that made it move. This caused the grinder to shift & make contact with the EE's finger, causing a laceration. EE had stitches & restrictions issued.
Employee was hit in the face by metal wedge that came out of drill pipe
IP was handling the wireline with hydraulic controls when the overshot became hung up on the bottom of the deck causing an uncontrolled release of energy resulting in overshot to contact the IP in the lower lip. This resulted in a laceration between the lower lip and chin and chipping one lower tooth.
