While cutting a block of metal from a sheet of steel, the block of steel was levered by a support block once it became free. The block struck the Miner's helmet and then the Miner's forehead which caused a cut on the forehead.

While operating a Komatsu D275 bulldozer, the miner ran over a rock that was sticking out from the ground. The dozer teetered over the rock causing it to jar against the ground. The miner felt discomfort to lower back and is being treated for the discomfort.

While operating a Komatsu HD785 haul truck during loading operations, a large rock fell from the Komatsu PC3000 backhoe that was loading the truck. The rock struck the tail end of the truck bed causing the front of the truck to bounce on the ground. The miner jammed leg causing discomfort to back and leg.

After a day of welding on Monday, July 17, 2017, the employee awoke at approximately 2:00 am the following morning with a burning sensation in their eyes. The employee reported to work on Tuesday, July 18, 2018 but left to see an eye specialist once they were open. The doctor diagnosed the employee with welders flash and prescribed eye drops.

While grinding a weld, a piece of ground metal entered the eye from below and underneath employee's safety glasses. EE was taken to an eye Specialist where the foreign material was removed.

EE was pressure washing a service truck when EE experienced severe pain in EE's right shoulder area. EE stopped what EE was doing, notified EE's supervisor and went home. The following day EE advised that the pain had not subsided. We scheduled an appointment with our panel physician for the same day.

Burning out steel plates in a CAT 777B haul truck. He was preparing the truck bed for relining when a shard of metal landed on his boot and melted through contacting foot. Treated at home for 2days with ointment. Today the area contacted was red ad irritated and subsequently we sent EE to the ER for evaluation. Recommendation three days with no boots or tight fitting shoes.

While welding on a bucket adjacent to another welder the employee was affected by the arc of the other welder. The employee's eye was scarred.

While using a torch to cut off an old wear strip, the wear strip broke loose striking the employee in the face.

While cleaning a Demag H185 excavator with a high pressure heated washer the employee accidentally waved the wand over his foot. The heated, pressurized water went through his rubber, steel tip boot burning his foot.

A CAT D9L dozer struck a loaded CAT 777 haul truck after traveling back in reverse. The dozer operator claimed to not see or hear the truck that was stopped and blowing the horn. The truck driver suffered a jarred back injury and the dozer operator did not claim to have been injured after the accident.

Employee was using hand tools to disconnect a hydraulic coupling on a Demag H185. There was residual pressure in the hydraulic system and he was sprayed in the eyes with hydraulic fluid when he loosened the coupling. The employee had his eyes flushed at the Hazleton General Hosp. emergency room and received follow-up care at an eye specialist in Hazleton.

While grinding metal using a handheld electric grinder the employee got a fragment of metal in his eye. He originally thought it may have been just dirt until the eye continued to bother him for a few days. He went to the ER on 1/12/09 and it was found to be a metal fragment.
