Employee was chiseling out dried and hardened cement inside the Paste Plant Mixer Hopper. While attempting to chip out a section of the hardened cement with an air chisel using the right hand, EE had their left hand behind the hardened cement. When the air chisel broke through the cement it contacted EE's left ring finger causing injury to that finger.

Injured worker was grinding on metal in the shop. They did not feel any pain but when they arrived home, they noticed what appeared to be a small piece of metal in their eye.

Injured worker was using an air lance at the paste plant to clean a hopper and felt tightness in their back after the shift. Employee reported the following morning that they still felt tightness in their back and needed to seek medical attention.

While fitting drill rod wipes to the drill pipe, driller moved head causing top section of drill pipe to fall 2" resulting in a pinch/crush injury to employee's right pinkie finger. This resulted in the amputation of pinkie finger.

Employee was using a pneumatic blowpipe to clean out accumulated dry tailings in a conveyor transfer chute. While inserting the blowpipe into access hatch the air control valve struck the side causing a significant increase in airflow. The blowpipe pushed back against the employee's grip causing their shoulder to extend.

Worker was on the Bench/Face of the COB #2 cleaning out drilled holes to prepare for loading of explosives. Worker was using a copper blow pipe with a 90 deg bend in it. While cleaning the hole, a piece of rock struck the worker in the face near eye brow of the left eye. Laceration to forehead, 3 stitches.

EE was reaming holes with a magnetic drill press with a 1-5/16" drill bit. The EE stuck his gloved hand near the bit to clean the cuttings away and glove became entangled lacerating right index finger and severing tendon.
