Employee was chiseling out dried and hardened cement inside the Paste Plant Mixer Hopper. While attempting to chip out a section of the hardened cement with an air chisel using the right hand, EE had their left hand behind the hardened cement. When the air chisel broke through the cement it contacted EE's left ring finger causing injury to that finger.

Operator was using a 6' pry bar to free 5" steel balls inside of discharge kibble bin. While attempting to the pry under the 5" steel balls, the balls broke free and forced the pry bar to move in the direction of the EE's side of the right face striking EE on that side of the face.

Employee was cleaning material buildup on the grizzly screen of a feed hopper. When attempting to step off of the grizzly bars EE's right leg fell through the grizzly bar openings (about 6" opening), causing injury to EE's right knee after hitting the metal bars.

When the oncoming shift was headed to the 2950 4503 to retrieve their equipment it was noticed that material had come down from the back. This is approximately 37 feet from the intersection and the fall is approximately 25 feet in length and 4 feet in Hight and 8 feet wide.

Injured worker was using an air lance at the paste plant to clean a hopper and felt tightness in their back after the shift. Employee reported the following morning that they still felt tightness in their back and needed to seek medical attention.

A power bump occurred across the site and caused damage to the cage proximity sensors at the shaft station. Hoist was not able to operate in automatic mode and the operator had difficulty transitioning it to manual.

During use the materials hoist stopped due to the brakes locking to the hoist drum. After investigation it was found that a failed brake control sensor caused the brakes to engage.

While fitting drill rod wipes to the drill pipe, driller moved head causing top section of drill pipe to fall 2" resulting in a pinch/crush injury to employee's right pinkie finger. This resulted in the amputation of pinkie finger.

Employee was using a pneumatic blowpipe to clean out accumulated dry tailings in a conveyor transfer chute. While inserting the blowpipe into access hatch the air control valve struck the side causing a significant increase in airflow. The blowpipe pushed back against the employee's grip causing their shoulder to extend.

Employee was walking down a flight of stairs from the Conditioning Tank down to Cleaner #1 in the mill when they slipped/tripped on a step causing injury to their left leg (knee, shin, and foot).

Employee was offloading materials in the service cage at the shaft collar. Forklift made contact with the door causing it to dislodge from track. Unable to fix within 30 minutes. Damage didn't interfere with the use of the equipment which is designated for materials. Operator is filing this under duress due to the ambiguous regulatory interpretation by District leadership.

While loading materials onto the service cage, contact was made with the door causing damage. The cage is used for either personnel or equipment and at no point were persons endangered or was the equipment damaged which interfered with its use. Operator is filing this report under duress.

Driller stopped work after identifying the driller helper began showing signs and symptoms of heat-related illness. As they were walking to the cool-down station the drill helper's symptoms escalated. So employee was taken to surface for eval and taken to local ER for evaluation, treatment and returned to work.

IP was using chain hoist to raise sump pump when the hoist stopped. IP jerked on the chain and felt a pain in shoulder. IP continued to work the remainder of the shift and the following day. Injury was reported to operator on 5/17/23 and accepted by WC on 5/30/23.

A process operator was pushing material with a dozer at the tailings stack when they exited their equipment to clear off accumulated rocks/materials from the track undercarriage. The employee turned to re-enter the dozer and tripped over a rock. The employee extended their left hand during the fall, taking the full weight of the impact onto their hand wrist.

An employee was struck by falling material while loading a face round in the 2896 heading. The employee (E1) was holding the pneumatic ANFO delivery hose and turned toward the face while another employee (E2) was operating the loading controls from the basket of the powder truck. E2 witnessed a falling rock strike E1 in the left shoulder and upper back.

Two employees were tasked with applying foam grout to the 2940 heading. While on a scissor deck a standpipe was sticking out and obstructing the deck. One of the employees found a ratchet tie-down strap and attempted to use it to pull the standpipe out of the way. While using the strap, the hook ends slipped and rebounded toward the employee, striking them on the left shoulder.

Worker was on the Bench/Face of the COB #2 cleaning out drilled holes to prepare for loading of explosives. Worker was using a copper blow pipe with a 90 deg bend in it. While cleaning the hole, a piece of rock struck the worker in the face near eye brow of the left eye. Laceration to forehead, 3 stitches.

While performing maintenance on a de-energized pump employee attempted to rotate the drive belt between the pump and electric motor by hand which resulted in right ring finger being pinched and lacerated between the belt and pulley. Employee received 5 sutures on 7/29/2022.

Employee was pulling weeds around the property on 6/18/22 having some lower left back pain. Resumed on 6/19/22, pain radiated to the left thigh and back. Taken to the ER. NSAID given. 6/20/22 returned to work and complained that the pain was still persistent. Taken to the ER. Given pain medication and 3 days off of work.

Got into the bed of work pickup truck to retrieve an item from the in-bed toolbox. When exiting pickup bed, grabbed onto the tailgate with hands. Lifted left leg over tailgate and onto rear bumper. Was lifting other leg over tailgate when fell to the ground on back with right ankle bent backwards. Not sure if foot slipped off bumper or what.

A mechanic was installing a bushing on the z bar of a scoop with a push plate by hitting it with a sledge hammer. While hitting the push plate with the sledge hammer a piece of the push plate flew off. The shrapnel flew and cut through the employees glove and lacerated the right hand knuckle.

Employee was carrying three 70 lbs jumbo drill steel from lay down to shaft on surface. The weight of the steel became too much to handle and employee tried to set the steel down. While bending forward employee's hand contacted the ground and the steel hit the top of employee's right hand causing a fracture.

The injured miner and two others were attempting to retrieve parts from a dismantled Galloway. The galloway was located at the 3000 vent shaft bottom. The three miners were lifting a metal sheave wheel from the galloway when it slipped from their hands pinching the right little finger of the injured between the sheave wheel and other metal.

Employee was getting ready to weld on the ball mill feed chute when the dog being used to push the plate tighter to the chute popped off and struck the employee between the eyes causing a laceration. The employee was wearing the proper PPE.

Employee injured left thumb while opening the rear axle cover on the Cat 1600 LHD. As they lifted the hood with right hand, left hand rested on top of the cab and as the cover passed the cab edge it pinched left thumb between the cover and the cab. This caused a laceration leading to the prescription of antibiotics.

When checking the upper limits switch on the service hoist, the hoist operator brought the service cage up into the headframe without de-clutching causing the counter weight on the other side to drop out of the guides. No one was in the cage, this was during pre-shift checks.

Employee started to raise the bucket to tram the loader away from the area when the bucket stuck in the raise position, the bucket was overloaded and the machine tipped forward causing the operator to be pitched over the front of the machine causing a dislocation of right knee.

Employee was walking to work area lost EE's footing and slipped on loose gravel. EE's immediate reaction was to reach and grab for a wire running along the Rib. In the process, EE dislocated EE's right shoulder.

DRAEO is providing short term consulting services at the client's operation. The injured employee was asked by a client employee to assist in removing a rock from a locked out conveyor. When setting the rock down, the injured employee's fingers were caught between the rock and the conveyor structure, ultimately resulting in the injury.

The East North Vent Shaft Rope was damaged when it was pinched between the VSM bucket and the shaft form while attempting to remove an oversized rock from the sinking bucket. The rope was thoroughly examined at 5:20 am and the hoist was taken out of service pending repair to the rope.
