Cutting with a torch, using a grinder, welder, and placing T-Bolts inside Kiln 7 cooler. Debris got into the maintenances workers eye. The maintenance worker noticed the eye irritation around 08:00 pm after being home for a few hours. Most of the work was performed overhead.

EE walks into the baghouse to see why the auger wasn't working. EE then walks back to the motor and sees the cover where the fan is looked like it had been beaten to death causing the fan not to run due to obstruction. EE was pulling on the belts to get the shaft to turn and that's when the motor kicked on causing EE's hand to get caught in between the pulley and the belt.

Employee reported employee had slag in left eye from working beside someone polishing boiler tube. Employee reports employee was wearing safety goggles. Employee presented to urgent care to have eye flushed. Employee now reports no discomfort to eye.

The EE was tramming the drill in the pit. The door was latched open. While moving, the door shut onto the EE's left hand. At the time, the injury consisted of minor lacerations. On 9/7/21, the EE's finger was swollen. We took EE to the doctor and EE had a minor fracture thus making the accident reportable.

EE was tightening the fasteners on the black plastic pipe (48"diameter) for the #1 kiln stack. EE had 7/8" wrench in left hand and a 1/2" impact with a 3/4" socket in right. When they pressed the trigger on the impact, the nut caught a burr on the bolt threads and the wrench spun out of hand and slapped EE across the knuckles.

Employee was using a hand held grinder on the seal flange on the # 4 feed end of the Kiln. A lot of material was airborne. Airborne material got into EE's shield and got past the safety glasses and into EE's eye.

Employee was performing maintenance on a dust pump. After replacing a butterfly valve and an auger bearing, the system was activated. The pump wouldn't discharge so the employee pressed the power switch again, the pump discharged sending fly ash out through the sliding hopper gate. Fly ash entered around the employee's safety glasses into eyes.

Employee was removing 3/4 threaded nuts from the screens rails. One got hung in the 3/4 Impact wrench socket. EE put left index finger into the 3/4 threaded nut, then accidently pushed the Impact wrench trigger. It rotated onto the tip of EE's finger

Employee was grinding on a flange on the #4 rotary kiln ring gear. Some dust got into EE's right eye. EE rubbed the eye probably imbedding the material from the dust. Later that day EE's eye became irritated.

Employee was using a 55ton hydraulic jack and a 6 inch I Beam to reline a rotary kiln hood. The jack slipped out of place, striking the employee to the face.

Employee was hooking up the welding trailer using the hydraulic lift from the tractor. Positioning the tong of the trailer and using the controls from the tractors hydraulic lift at the same time, the tong receiver on the trailer slipped off the ball hitch smashing EE's index finger.

Employee was cutting metal using an Oxyacetylene torch. It appears some rust was behind the metal employee was cutting, hot molten metal splashed back into employee's face. A small piece of molten metal worked its way behind employee's cutting goggles and attached to employee's eye.
