EE was roof bolting and when the drill head dropped down, the steel fell out of the hole, striking him in the head. EE was wearing hard hat. There were no lacerations or cuts. Not a serious injury.

Injured stated he was drilling a hole in the mine roof when he hit a crack and let his drill steel come out of the chuck stirking him on the fingers.
INJURED WAS BOLTING THE MINE ROOF WHEN A PIECE OF DRAWROCK MEASURING APPROX 18" IN DIAMETER BY 3" THICK FELL FROM BETWEEN THE LAST ROW OF ROOF BOLTS AND THE ATRS STRIKING HIM ON THE FOREARM REQUIRING STITCHES.
INJURED STATED HE TWISTED HIS KNEE WHILE BOLTING THE MINE ROOF. INJURED WORKED UP TO AND INCLUDING 10-22-03 AND HAS NOT WORKED SINCE.

INJURED STATED THAT AS HE WAS BOLTING TOP HE SLIPPED AND TWISTED HIS BACK. INJURED HAD NO LOST TIME UNTIL 10-19-01.

INJURED WAS REPAIRING THE TAIL ROLLER FOR THE CONTINUOUS MINER WHEN THE ROLLER ASSEMBLY SLIPPED AND CAUGHT HIS THUMB BETWEEN THE MINER TAIL AND ROLLER ASSEMBLY CUTTING IT REQUIRING 5 STICHES.
INJURED HAD EXITED THE DECK OF THE MOBILE BRIDGECONVEYOR HE WAS OPERATING TO RETRIEVE A CHAIN. AS HE WALKED TOWARD THE FRONT OF THE MBC, THE NEXT MBC INBY BEGAN TO BACK UP PUSHING INJURED'S M BC AGAINST THE LO-LO BELT STRUCTURE CATCHING THEINJURED'S LOWER LEGS BETWEEN THE MBC AND THE LO-LO BELT STRUCTURE.
THE INJURED RAISED THE HEAD OF THE CONTINUOUS MINER TO KNOCK DOWN DRAW ROCK WHEN A PIECE FELL,HIT ON THE MINER, BOUNCED AND HIT OPERATOR.
THE INJURED WAS TRAMMING THE ROOF BOLTER UP INTO#3 HEADING WHEN HE STEPPED ON A ROCK & HIS FOOT SLIPPED & WENT UNDER THE TIRE. THE MACHINE ROLLED ON TOP OF HIS RIGHT FOOT.
INJURED STRUCK HIS HEAD AGAINST A METAL BAR LOCATED IN THE CAB OF THE DOZER AS THE DOZER DROPPED OVER AN 8' EMBANKMENT WITH A SUDDEN JAR. INJURED REQUIRED STITCHES TO THE HEAD AND RETURNED TO WORK WHEN THE STITCHES WERE REMOVED WITH NO RESTRICTIONS.

THE INJURED WAS ALINING A STEEL STRAP WHEN IT SLIPPED AN STRUCK HIM ACROSS THE FOREHEAD CAUSING A LACERATION. 2 STITCHES SUTURES.
