A mechanic was working on a Sullair air compressor engine oil leak with the engine running. EE had an inspection mirror in EE's left hand and a flashlight in EE's right hand. EE observed that the engine was not leaking anymore so EE pulled EE's hands back. The fan caught EE's left hand and pulled it into the fan causing injuries to EE's left hand.

Employee was cutting through a metal wall on 902 dragline, during this process the employee flinched. This caused the grinder to jump, and the cut-off wheel came into contact with employee's finger. Although gloves were in use, the injury still required sutures.

A welder was using an air chipping hammer to remove picks on the primary crusher at the Coal Handling Facility, when the electrician that was working with EE felt something hit EE's forehead. The electrician was 5 feet away from where the work was being done. The employee was contacted on the left side of EE's face above EE's eyebrow.

On March 7th, 2024, a dragline ground person was in the process of pushing in a ramp off the bench to access the pit floor for cleaning. During the process of pushing in the ramp, the dozer blade caught a rock or frost edge. The dozer turned sideways, overturned and then came to rest on its tracks. The employee was wearing the seatbelt at the time of the incident.

On May 31st, 2023, an employee was working to remove the crowd brake on 935 electric shovel to replace broken bolts. When the final bolt was removed, the stored energy in the crowd brake caused the brake to rotate. When the brake rotated, the employees arm came into contact with it. This contact caused lacerations to the employees' arm, which resulted in stitches.

A Technician was working on crack repair in the revolving frame on a Marion 8750. While grinding, a small metal fragment contacted the employees right eye while wearing safety glasses and a welding hood.

Employee was performing a welding/grinding repair on a Dragline bucket. Later in the shift the employee's eye became irritated. The eye was flushed several times and nothing was identified in the eye. The next morning the eye was still irritated. An eye doctor identified a metal sliver in the eye. Proper PPE was being utilized when welding/grinding work was being performed.

An employee was in the process of starting to line bore. During this time the employee was shaking a can of spray lubrication to prepare to lube the cut. During this action the employees hand encountered the boring head which then pinched EE's right thumb against the bearing plate. This caused a partial amputation of the right thumb, and a broken right ring finger.

Welder was welding on 6040 Bucket in the shop all day with no complaints of eye irritation. Welder finished shift, went home and woke up from sleeping at 10:30 pm complaining of eyes irritating. Took employee to doctor where employee was diagnosed with arc flash burn.

Technician was installing final drive on the 6020 excavator. While using the hytorq pump and wrench to torque bolts, EE caught thumb between the bolt and the hub, causing laceration to left thumb.

Employee was wearing safety glasses & face shield, but at some point during grinding/air arcing a particle contacted the employees right eye. The eye was flushed & employee finished the shift and went on days off. On 8/28/18 the eye was bothering so employee went to eye doctor who removed a small particle.

Individual welded majority of Monday with no signs of injury and completed full shift. Early Tuesday morning, individuals eyes were burning. Went to doctor and confirmed individual burnt cornea from welding. Eyes were more sensitive due to lasik surgery previously had and was unaware long periods of time welding would effect EE. Doctor prescribed prescription eye drops.

Employee stated that while using a 250rpm mag drill to drill holes for bolts in the baseplate of the escape tube fan he believes he incurred fragments into his eye. The employee had utilized his hand to wipe sweat from his face during the task possibly resulting in the fragments entering his eye causing an corneal abrasion to his left eye.

An employee was cleaning mud and ice in the Shop with a Wrangler when the cutting edge got hooked on the rails that are embedded into the concrete floor. This caused the Wrangler to stop abruptly and the employee hit his lower lip on the steering wheel. The employee received a laceration that required stitches. The employee was wearing a seatbelt.

Employee was grinding out a weld area on the mast of the dragline. Employee was wearing all his personal protective equipment. Grinder trigger was not engaged but disc was still spinning when grinder slipped in employee's hand, disc contacted left index finger cutting tip. Finger required 4 stitches, employee returned to work.

Employee was grinding out an air arc cut for repair on a front end loader. Employee was wearing safety glasses and a face shield. The grinding particle found its way under both and contacted his right eye. The work area was fairly open, but had some potential for particles to deflect around. The employee was taken to eye doctor who removed the particle. Employee returned to work.

After checking a pump, Employee felt something hit his L arm. A witness said that as the dragline swung toward the pit after dumping, a rock fell from the lip of the bucket. Considering that the pump was located near the swing radius of the dragline, it is believed that centrifugal force caused the falling rock to hit the operator. Employee's L arm was cut and elbow was fractured.

The employee was standing between the tri-structure and fairlead tower. He was in the process of removing a piece of keystock from the back leg of the fairlead tower. The employee was using a 5 inch grinder and a 5 inch wheel to remove the tack welds that were holding down the keystock. During this process the grinder kicked back, and struck his right leg causing a laceration.

I WAS UP ON LIFT UNWINDING SPRINGS TO CHANGE OUT SPROCKET ON OPENER. I WAS TAKING OUT BOLTS AND NUTS WITH IMPACT WRENCH AND BACKUP WRENCH, WHEN I TOOK OUT FIRST BOLT AND NUT SPRING BEARING PLATE SPUN AROUND SHAFT AND CAUGHT MY WRIST AND CUT IT.

Driller was drilling undeveloped area to identify location of coal seams. Moving to another boring location when rig popped out of gear and began rolling down an incline. Could not get into gear and brakes could not stop momentum. Driller ran rig into a tree, injuring wrist and knees.

Employee was grinding welds between 2 safety posts that come together. The grinder jammed between the post and kicked back hitting the employee in the welding shield breaking the hood. The clear lens and plastic on the hood broke causing lacerations and scratches on the employee's nose. The employee was taken into a clinic where he received 6 stitches.

Emp noticed an eye irritation in his right eye on the evening of 2/3/12. The emp was fitting the top rim angle on a fuel tank earlier that day on a mine site. The emp did not seek medical attention until 2/6/12. An optometrist diagnosed that he had developed a scratch on his eye but was released to come back to work that day with prescription antibiotic eye drops.

Mechanic was removing roller frame from bulldozer in shop with shop crane. While setting frame on floor their left fingers were caught in a pinch point under track rollers.

A piece of slag fell into EE's left boot while welding. EE waited until weld was complete to remove boot and retrieve slag. Dime sized burn became infected and required debridement on 6/17/10.

Employee was operating a demolition saw. Employee was cutting a vertical concrete wall when the saw "kicked back", striking the employees faceshield, knocking his faceshield up, and cutting his lip and nose.

EE was trouble shooting air conditioning system on a Cat 24H motor grader. When checking air flow in an air duct, the employee did not realize the exposure to a moving fan. When retracting his hand it caught the fan, injuring two fingers. The finger tips were cut and fractured requiring medical treatment.
