EE WAS INSTALLING BOLTS ON HELPER SIDE OF A DD015 FLETCHER BOLTER IN THE #2 FACE. AS HE WAS PUSHING UP THE BOLT, A LOOSE PIECE OF DRAW BROKE AROUND THE BOLT (APPROX SIZE 1' X 6" X 3"), FELL AND STRUCK EE ON HIS HAND. BROKE EE'S RING FINGER.
Rock falling hit him on top of right foot. Witness stated he was on the open side when this happened and when he got to him he had foot out under rock.
ROOF BOLTER OPERATING HELPER SIDE OF THE DOUBLE HEAD FLETCHER. WAS INSTALLING HIS SECOND BOLT IN THE FIRST ROW IN THE #7 ENTRY WHEN A LOOSE ROCK MEASURING 5' X 5' X 0 TO 5" THICK BROKE OFF AT THE EDGE OF THE ATRS AND FELL STRIKING OPERATOR ON THE RIGHT LOWER LEG.
BOLTER OPERATOR WAS INSTALLING HIS 2ND BOLT WHEN A LOOSE PIECE OF DRAW ROCK APP 0-3" THICK 3' LONG & 2' WIDE FELL STRIKING HIM IN THE BACK & BRUISING HIS RIBS. HE WAS SENT TO THE HOSPITAL, TRE ATED & RELEASED. RETURNED TO LITE DUTY NEXT DAY.
EMPLOYEE WAS BOLTING TOP WHEN THE DRILL BIT BROKE. HE CONTINUED TO PUT UP PRESSURE ON THE DRILL STEEL WHICH BROKE AND HIT HIM ABOVE THE RIGHT EYE.
MAN LEAN BETWEEN THE BOOM AND THE SYSTEM OF THE MACHINE TO REMOVE DUST HOSE, ACCIDENTLY HIT THE UP CONTROL OF THE BOOM WITH HIS LEG AND PIN HIMSELF BETWEEN THE ATRS AND BOOM OF THE MACHINE.
