EE & others were replacing the top belt on the #2 belt press. Takeup assembly was secured with a 3/4 ton chain come-a-long, both air regulators were backed off so as not to apply pressure & a stand with a 2"x12" steel pipe was holding the new belt. The air valve was opened & the takeup swung forward, broke the chain & the pipe struck the injured on left ear which required stitches.

Operating drill, steel got stuck. Asking for assistance, employee was using the wrench as co-worker had controls. Wrench grabbed, forcing his hand between wrench and steel.

Employee stated on 8/03/06 that he had pulled a muscle in his lower back while tightening lug nuts on a truck tire. He notified company of the alleged incident after receiving pay minus a previous pay advance which the company paid because he was considered a voluntary quit. His 1st Dr release for "illness" from 7/25 thru 7/29; 2nd release from 08/03 thru 8/07 "no remarks".

Employee was checking condensor blower motor that was inoperative when his right hand came in contact with a fan adjacent to the one he was checking. The result was lacerations to his right index and middle finger requiring stitches. The employee did not miss any days worked - medical reportable only.

INJURED WAS USING AN OVERHEAD HOIST AND A GRAB LIFT TO REMOVE A 2'X4'X1/2" STEELPLATE FROM THE STORAGE RACK. HE RAISED THE PLATE APPROX 5' FROM THE FLOOR AND WAS ATTEMPTING TO ROLL THE HOIST WHEN IT REFUSED TO MOVE. HE TRIED SEVERAL TIMES TO PULL THE HOIST WHEN THE PLATE FELL OUT OF THE GRAB, STRIKING AND FRACTURING HIS LOWER RIGHT LEG

INJURED WAS ATTEMPTING TO CANGE A BROKEN SPRING ON A RUBBER TIRED MANTRIP. HE RAISED THE MANTRIP WITH A FORKLIFT & WHEN HE WAS LOOSENING THE SPRING BOLTS, THE MANTRIP TEETERED & THE END DROPPED CATCHING INJURED'S RIGHT HAND BETWEEN THE IMPACT WRENCH AND THE GROUND. WHEN HE PULLED HIS HAND FREE, HE LACERATED THE BASE OF THE MIDDLE FINGER. TRANSPORTED TO ER & RECEIVED 7 STITCHES.

WHILE SHOVING WITH A D11R DOZER, A ROCK ROLLED UNDER THE BLADE & THE LEFT TRACK ROLLED ONTO THE ROCK. THE DOZER SLID OFF OF THE ROCK, CAUSING A SUDDEN FORWARD MOTION TO THE EE, RESULTING IN AN INJURY TO HIS LEFT SHOULDER & NECK. THE EE CONTINUED TO WORK WITH NO MEDICAL TREATMENT UNTIL 4/5/04 - AT THIS TIME INCIDENT BECAME REPORTABLE.

EE WAS ADVANCING CONTINUOUS BRIDGE SYSTEM AND WAS POSITIONED AT THE END OF THE TAILPIECE. THE CONTINUOUS MINER OPERATOR REVERSED THE CONTINUOUS MINER, FORCING THE EDGE OF THE DECK ON THE BRIDGE INTO THE TAILPIECE, WHICH RAISED IT INTO THE MINE ROOF. THE EE STRUCK THE MINE ROOF WITH HIS HEAD, CAUSING A STRAIN TO THE NECK.

Mechanic was talking old cutting edges, held on with bolts and nuts, off of d5h cat dozer blade. use an air wrench 3/4 drive to take the nuts off he used his thumb to hold the hand bolt down into a countersunk hole, while air wrench spin. the bolt at high speed the bolt came out of hole pinning.

INJURED WAS HOLDING A TORQUE MULTIPLIER UNDER A DOZER BLADE ON A NUT, WHILE THE MECHANIC APPLIED THE RATCHET ON THE INPUT SIDE. THE HANDLE OF THE MULTIPLIER WAS BRACED AGAINST A STEEL RIB UNDE R THE BLADE. THE HANDLE SLIPPED PFF THE RIB CATCHING THE INJURED'S THUMB BETWEEN THE HANDLE AND THE DOZER BLADE.

INJURED AND SUPERVISOR WERE STANDING OUTBY THE CONTINUOUS MINER WATCHING COAL BEING LOADED INTOTHE COAL HAULER. A PIECE OF ROCK HAD FALLEN ONTOHEAD OF MINER. WHEN THE PIECE OF ROCK CAME BACK THE CONVEYOR CHAIN, IT FELL OFF AND STRUCK INJURED ABOVE THE RT. KNEE. SUPERVISOR HAD SEEN THE ROCK AND PULLED INJURED BACK.
