A raw coal pile of approximately 100,000 tons had been stockpiled over a period of many months. The fringes of the pile, because of a lack of compaction, became hot and eventually started to burn throught the crust of the pile.

Officer on foot patrol at prep plant-inside. She stepped over a cat that was laying on the floor, bumped into floor fan and fell down, hurting her knee.

EE & others were replacing the top belt on the #2 belt press. Takeup assembly was secured with a 3/4 ton chain come-a-long, both air regulators were backed off so as not to apply pressure & a stand with a 2""x12"" steel pipe was holding the new belt. The air valve was opened & the takeup swung forward, broke the chain & the pipe struck the injured on left ear which required stitches.
Notified on 2/1/07 that claimant was awarded permanent partial disability for occupational pneumoconiosis.
Right hand between wrist and knuckles-eas pulling belt rubberon to structure using 1"" rope attached to a truck. was in process of rehooking when came loose and struck hand.

Employee scraped his knee while coming off the steel. He had a mild abrasion (similar to a carpet burn) and continued to work until 7/7/06 when he went to the doctor for an infection of the knee. Management was not made aware of his medical visit until 7/19/06 when notified by comp carrier.
INJURED & CO-WORKER WERE REMOVING A SPEED REDUCER FROM THE END OF A MAGNETITE AUGER. AS THEY ATTEMPTED TO REMOVE THE REDUCER USING A CHAIN HOIST, THE REDUCER BECAME STUCK. THE INJURED USED A SLATE BAR TO PRY LOOSE, IT SWUNG & HIT HIS RT. KNEE CAUSING DAMAGE TO A LIGAMENT.

INJURED WAS USING AN OVERHEAD HOIST AND A GRAB LIFT TO REMOVE A 2'X4'X1/2"" STEELPLATE FROM THE STORAGE RACK. HE RAISED THE PLATE APPROX 5' FROM THE FLOOR AND WAS ATTEMPTING TO ROLL THE HOIST WHEN IT REFUSED TO MOVE. HE TRIED SEVERAL TIMES TO PULL THE HOIST WHEN THE PLATE FELL OUT OF THE GRAB, STRIKING AND FRACTURING HIS LOWER RIGHT LEG
Employee turned the truck over while dumping at the stockpile. He complained of right shoulder pain. Ambulance took him to the hospital to get checked out.
EMPLOYEE WAS ATTEMPTING TO OPEN THE ENGINE DOOR & HAD HIS RT. HAND ON TOP OF THE DOOR. WHEN INJURED OPENED THE LATCH WITH HIS LEFT HAND & PULLED, THE END OF HIS RT. RING FINGER GOT PINCHED BETWEEN THE DOOR & THE FRAME CAUSING THE INJURY.
NOTIFIED ON 6/7/05 THAT CLAIMANT WAS GRANTED A 3.4% PERMANENT PARTIAL DISABILITY AWARD FOR HEARING LOSS.

INJURED WAS ASSISTING OTHER EMPLOYEES WHILE INSTALLING A 6"" POYURETHENE FRESH WATER LINE. AS THE PIPE WAS BEING PULLED TO THE POND, THE INJURED STEPPED OVER THE PIPE. THE INJURED CAUGHT HIS RIGHT FOOT ON THE PIPE CAUSING HIM TO FALL TO THE GROUND FRACTURING HIS RIGHT FEMUR.
INJURED WAS CLIMBING UP ONTO THE DOZER & HAD A FLASHLIGHT IN HIS RT. HAND. HE STEPPED UP ONTO THE TRACK & ATTEMPTED TO GRAB ONTO A HAND HOLD WITH HIS RIGHT HAND BUT MISSED CAUSING HIM TO FALL APPROX. 4'TO THE GROUND. HE FELL ONTO HIS LEFT SIDE CAUSING BRUISING TO HIS RIB AREA.
Truck driver pulled under refuse bin to load his vehicle. While exiting, he slipped on the step of the vehicle and lost hand contact. After regaining hand contact with the truck door he pulled a muscle in his neck area. physician at summersville memorial hospital documented a return to work date of 4/2/04.
