IE was cutting bottom and slowly trammed miner to the right. The miner cable loop rolled up and struck IE in the back of left knee. IE continued to work until 08/15/2025 where the MRI showed they had a slight tear in their meniscus. IE chose to have surgery to repair.

IE was using an electric grinder that became entangled w/IE's welding coat sleeve. Before the grinder stopped it cut IE's wrist which needed a few stitches to close.

IE was standing on relay bar clevis when the pan was pushed pressurizing relay bar and pushing IE's foot underneath step at Head gate smashing EE's foot causing a fracture to EE's left big toe. IE continued to work EE's normal shift from 12-12-2024 up to 12-31-24 when decided needed to be cast and off work.

IE went to the tailgate side of shear, opened the cabinet door to run the tailgate cowl off the valve bank While operating the cowl, the cabinet door closed striking their right forearm. IE was diagnosed with a hairline fracture. They returned to work their next shift

Employee was learning how to lower a shield on LW tear down. The return gob side PTO line was missing staple holding hose in place. When the function was pushed, the hose came out of the PTO spraying emulsion into IE's eye.

The employee was using a 6-inch electric handheld grinder with a cutoff wheel. The employee was cutting a steel bar grading walkway when the grinder kicked back and struck EE's right thumb. The grinder caused a laceration just above EE's right-hand thumb knuckle that required 5 sutures to close.

Injured had been using a hot water pressure washer and decided to change out the wand for a tip with a wider spray pattern. While changing the wand, injured accidentally pulled the trigger and shot high pressure water thru boot and into the top of foot.

A continuous miner operator had completed a cut and was in the process of tramming to the next place. While backing the miner out, the operator pinned self between the coal rib and the tail of the continuous miner. EE was able to free self using the remote control.

IE was hit by a rock in the lower right leg above the ankle, clearing rock of the duct work of a miner. The IE continued to work thinking the injury was not anything serious, but recently claimed it has been getting worse. The IE was seen by an Orthopedic doctor and was taken off work on 09/01/23 for a small tear in the calf muscle and placed in a walking boot.

While bolting in 3 entry on 406 bolter the bolter helper was setting the ATRS when the left side bolter operator boomed in and smashed the helper's finger between the frame over the levers and the bolter boom tray. Left middle finger is the injury.

IE was bolting on the right hand station. IE was bolting in the roof on a corner at an angle. IE had the drill box all the way up. On the way back down IE had the left foot below the drill box. Once IE got the drill box toward the bottom IE smashed the foot. IE has a broken left, big toe. IE is in a boot and on crutches. At this point IE is seeking orthopedic care.

IE was handling cable for the roof bolter backing the machine out of entry 2. The cable became tight and hit the IE below the knees, causing them to fall. The IE reached out with arms to catch themselves as they hit the floor with both hands then rolled on to the left shoulder. As of 9/26/2023 IE turned in voluntary resignation.

IE was struck in foot by a rock during the mining cycle. The rock came over the sloughing plate. IE was taken in for X-rays and they were negative. IE was able to return to work and was waiting for MRI. The MRI was received on April 5th, 2023. MRI showed a minute fracture in the foot and on April 5th was when it became reportable/lost time.

IE was roof bolting on Unit 3. As the IE was installing a 6' fully grouted bolt, the bearing plate wasn't orientated in the right direction. The IE reached up with the right hand to position the plate, simultaneously pushing the bolt into the hole, pinching the tip of the right index finger between the plate and roof.

IE was drilling a hole to install a roof bolt. The drill steel became damaged and struck the IE in the right hand between the thumb and index finger causing a laceration. The IE received 5 sutures to close the laceration.

Replacing a dragline wear liner the employee was unable to see a developing crack on the opposite side of the plate due to a welding hood. A preexisting weld failed to hold plate due to age causing the plate to fall & injure the employee. Employee was found to have a fractured toe and a laceration resulting in sutures. Employee returned to work for employee's next scheduled shift.

IE was removing jam nuts from the cable boat for electric shovel. The truck crane holding the wire rope was moved without IE knowing. The wire rope struck the IE's left pinky finger. IE had a laceration which required 5 stitches to close.

The IE was using a hand drill to bore out a broken bolt to a tram lever on a roof bolter. As the IE applied pressure to the drill, the 3/16" bit scattered causing a fragment of metal to become logged in the corner of the right eye. The IE was transported to the Sparta Community Hospital ER where medical treatment was administrated, to care for the eye.

On 12/27/21 while drilling a hole to install a 6' roof bolt. The IC had a premade roof bolt leaning against the drill boom tray. Unknowing the location of the base of the bolt, as the IC lowered the drill boom it struck the bolt causing it to strike the right knee. The IC continued to work until placed on restricted duty by and Orthopedic doctor on 1/4/22.

On 09/22/21 IE was roof bolting on Unit 2. While installing the 3rd row of roof bolts, IE swung the drill boom towards the right rib. The IE set the drill boom. It is believed that the foot of the drill boom was either positioned on a rock or weak floor material. The base smashed the rock/weak floor and lowered the drill boom, striking the IE in the hard hat.

IE was moving #31 remote controlled bolter forward from entry 6-7 to entry 5. When the bolter went through a curtain in 6-7, the cable got caught on the bolter. As the IE went to reach for the cable to move it, EE believes they hit one of the tram buttons causing the machine to shift and pin themselves between machine bumper and the coal rib. IE was treated for a fractured pelvis.

IE was air arcing a frame crack on an end dump. IE was wearing leather welding gloves, leather welding jacket, and leather welding apron. A hot piece of slag bounced off something and landed on IE's pants just about the boots. It burned through the jeans and fell into IE's boot, giving IE a 2nd degree burn on the ankle.

IE was repairing cracks in shovel bucket with another welder. They began welding at 8am and took turns until 3. They were both wearing 3M auto-darkening air-hoods. Later that night EE was diagnosed with arc flash from the intermittent welding

Employee began to feel discomfort in left eye. Employee was taken to the optometrist where a small rust ring was removed. No object was imbedded. A prescription for a steroid was given.

Empty coal hauler turned in outby of entry #6, continuous miner operator in the face of #6 entry moved behind the tail of the miner to check sights. Empty hauler proceeded to back towards miner for loading, miner operator remained behind tail. Miner operator became pinned between tail of miner and tailgate of coal hauler. Miner operator suffered severe injuries to upper R leg.

While a row was being bolted the I.E. stated that they rested their right hand in a pinch point and became distracted looking at supplies needed for the next bolt row. It was at this same time that right side operator retracted the boom on the bolter, which caused the boom mounting pin assembly to come to rest against the stop

IE was standing beside a slack pile of roof bolter cable as it was being reeled up. The cable became tight and made contact with the top of EE's left hand and wrist. EE received a fracture of the scaphoid bone of the left wrist.

Injured Employee (I.E.) was tramming a 12CM12 continuous miner (company #317) into the face and positioned on the right side. I.E. was found pinned between the continuous miner tail and the rib. Current known injuries: 7 broken ribs (right) 4 broken ribs (left) and a collapsed lung. I.E. is stable.

IE was adjusting a fan in the surface shop to direct air towards where EE was working to help keep EE cooler. The IE grabbed a hold of the side of the guard which surrounds the fan to swivel it, one of the fan blades came in contact with the guard causing a piece of the fan blade to break off, causing a portion of the fan blade to go through EE's glove into the palm of EE's hand.

Employee was in the process of installing the extension drill steel needed for drilling an 8ft bolt hole. The extension was coupled with the starter drill steel and the drill head was brought up to insert the extension into the drill head. The steel did not align correctly and kicked out into the injured employees thumb
