Employee was securing barge with tugger wire to timberhead and during this process accidentally hit the remote tugger winch on/off switch with his knee. As a result, the employee pinched his left hand fifth digit finger between the wire and timberhead. Employee fractured the tip of left hand fifth digit finger as well as received 3 stitches.

EE was cutting bushing for pin on backhoe, when the grinder hung up and kicked back, striking EE below right elbow. Required 2 stitches just below right elbow.

Employee was dropping a loaded barge down river. While doing this the employee was climbing the ladder access to the dock barges and at the same time the tipple operator was starting to hoist up the barge cable. EE's foot was caught between the ladder rung and the winch cable, which resulted in EE's left foot being broken.
Operating roof bolter, drill steel got stuck in hole. Operator reached out to hold steel and was going to hit the reverse rotation on drill. Instead he hit forward causing steel to come out and strike the back of the operator's hand causing a break in the left wrist bone.
Employee was operating roof bolting machine and felt pain in lower back.
Drilling a hole to install roof bolts using a 6' steel, he used his wrench to torque bolts to drill a deeper hole and steel bent. He reversed the drill rotation and was holding the steel in his hand when he hit the reverse lever. Wrench came off steel, hit his left arm causing a simple fracture of the forearm.
Employee was spraying plaster on bratttice, the end of the spray nozzle blew off striking him in the left eye.
Employee was bolting top and twisted right knee.
Employee was removing a bit lug from a continuous miner, a piece of metal struck employee in right leg.
Employee was drilling a hole for a tail piece, slipped and fell back on his left hand resulting in a sprain.
Employee was tramming the roof bolter up an incline and had his arm around the canopy post of the deck of the roof bolter. The roof bolter ran into the rib and cut his right arm.
WAS ROCK DUSTING WITH A SCOOP BUCKET DUSTER, HIT ROCK BOUNCED CAME DOWN ON SELF RESCUEER, HIT LOWER BACK
Employee was operating roof bolter and a piece of draw rock fell striking employee on left leg.
Employee was clearing brush from the river bank at the upper bin and during this time the chainsaw he was using accidentally came in contact with his upper left left (thigh) resulting in a laceration. The laceration required 38 stitches to close.
Employee was running roof bolter and started to drill a hole and a piece of draw rock fell from top cutting his face. (Required 6 stitches)

EE WAS INSTALLING ROOF BOLT AND THE HYDRAULIC HOSE PROVIDING PRESSURE TO THE ROTATION OF THE DRILL HEAD RUPTURED SPRAYING HYDRAULIC OIL IN HIS FACE SOME OF THE OIL HIT HIS EYES EVEN THOUGH HE WAS WEARING SAFETY GLASSES.
HE BACKED THE CONTINUOUS MINER OUT TO SET OVER TO THE OTHER SIDE. WHEN HE SPLIT HIS CATS THE MINER SLEW OVER AND CAUGHT HIS SIDE AND LEFT AGAINST THE RIB. AND THE HEAD OF MINER WAS SITTING ON HIS FOOT "LEFT". NO BITS WERE IMPALING HIS BODY OR FOOT. HE REFUSED MEDICAL ATT.
EMPLOYEE WAS GRINDING ON A BOLT HEAD. THE GRINDER SLIPPED OFF AND STRUCK HIM ON THE ARM WHILE THE GRINDER WAS RUNNING, CUTTING HIS ARM ABOVE HIS GLOVE.
REMOVING LUG BOLTS FROM SCOOP TIRE FELT PAIN IN LOWER BACK.
EMPLOYEE WAS INSTALLING ROOF BOLTS IN #8 RIGHT XCUT. DRAW SLATE FALL ALONG THE RIB STRUCK HIM.
EE WAS OPERATING A REMOTE-CONTROLLED CONTINUOUS MINER WHEN A ROCK ROLLED OFF THE RIB AND STRUCK HIS RIGHT ANKLE.
EE WAS BOLTING TOP WHEN DRILL STEEL BROKE STRIKING HIS RT. THUMB. EE CONTINUED TO WORK, WAS ADVISED ON 4/12/02 THAT A VERY SMALL HAIR LINE FRACTURE WITH NO DISPLACEMENT.
EE WAS DRILLING TEST HOLE AND LAID HIS HAND ON BOOM OF ROOF BOLTER AND RAISED THE BOOM UP CATCHING HIS RT. 2ND FINGER ON THE RT. HAND.
EE WAS WATCHING CRAWLER DRIVE GEAR TO ALIGN IT FOR WELDING WHEN PIECE OF METAL ENTERED HIS EYE (HE WAS WEARING SAFETY GLASSES). PRESCRIPTION MEDICATION WAS USED, AS USUAL.
EMPLOYEE WAS DRILLING A HOLE WITH A PIECE OF DRILL STEEL WITH A BOLT WRENCH ON THE BOTTOM. THE WRENCH SLIPPED OFF STEEL STRIKING EMPLOYEE ON THE HEAD.
EE WAS USING A WIRE BRUSH WHEEL ON AN AIR-OPERATED DIE GRINDER TO CLEAN A GEAR CASE WHEN A PIECE OF WIRE STRUCK HIS LEFT FOREARM. HE WORKED AT HIS REGULAR DUTIES UNTIL 12-12-2001, WHEN THE PIE CE OF WIRE WAS REMOVED BY A PHYSICIAN. HE RETURNED TO WORK AT REGULAR DUTY ON SAME DATE.

EMPLOYEE WAS USING CUTTING TORCH TO TAKE DOWN BELT STRUCTURE WHEN HE CUT BOLT HOLDING STRUCTURE TO ROOF, STRUCTURE FELL, STRIKING EE ON TOP OF RIGHT FOOT FROM BEHIND SAFETY TOE AND BROKE BONE ON TOP OF FOOT. EE HAD BEEN REMOVED FROM WORK FOR UNRELATED MEDICAL REASONS ON 7-11-01. FRACTURE DISCOVERED 7-19-01.
