Showing 50 of 145Employee was working in ball mill while other employees were using a jack hammer to remove a bolt. When they hit the bolt, feed fell between his safety glasses and eye. He flushed his eyes, but by Wednesday, 2-21, his eye was irritated. He was taken to the doctor, a small speck of metal was removed (not embedded), given antibiotics and returned to work.
Employee was operating a dozer, another employee was operating an end dump and was given the signal to dump by the spotter. The spotter then realized that the dozer had moved but the material was already falling on the dozer. A rock landed on the dozer operator's hand, causing a laceration.
Employee removed the choker from the crane lowering the impactor at the crusher. The impactor shifted causing it to fall and caught his ankle between it and the old headnut on the floor. This resulted in a contusion to his left ankle.
Steel plate was pressed down and tacked in place. Employee then took position on top of plate to continue welding. While sitting on plate performing job duties, the tacks failed causing a sudden release of stored energy jarring employees lower back. EMPLOEE DID NOT START FEELING PAIN UNTIL 2/20/07 AND REQUESTED TO BE TAKEN TO THE CLINIC.
Drill string hung up and then dropped, smashing employee's right thumb between water swivel and chuck jaws.
While burning the fastners off the hold down bracket on the thrust block on a 12 line for the Tailings Dam at the 7-C deep well south of silver basin, an employee was adjusting his torch after lighting it when it slipped and burned the top of his right hand under the glove.
The operator of the 65 shovel was loading the T557 and on the third dipper, the dipper clipped the duck tail.
Employee was welding on a service truck using an older welding hood. He did not see any bright flashes through the hood while welding through out the day. When he left for home he was not having any problems with his eyes. Around 9:00 pm his eyes started burning so he put in some eye drops. He woke up around 2:00 am and his eyes hurt and he could hardly see.
Employee was running an impact in one hand and had a wrench in the other hand. Hit the trigger on the impact and the wrench jumped back against left wrist. Didn't have pain at first. As time went on started to notice pain and discomfort in left wrist while doing heavy pushing or pulling.
While arc-cutting sticks on shovel, employee's pant leg caught on fire, burning his lower part of his right leg.
While moving 12? feed line off of the top of the 4 west dam (south face). Dozer was pushing the pipe off the top bank away from the valve. Pipe struck another employee as it was being moved.
Employee was drilling with a hammer drill when the bit got stuck causing the drill housing to rotate twisting his finger.
At end of shift on 11/17/06, employee reported that he had jarred his neck backing out of his first pass of pit cleaning and hit a rut. He refused examination that day. The next day, reported worsening pain, was taken to the ER and taken off work. CURRENTLY ONGoing.
The operator of the 538 haul truck was trying to dump out on the MOP dump. When the bed of the truck was partially up it fell back down with muck still in it. The driver informed the 402 tower and ERRT was dispatched. The driver had pain in her back and she was taken to the clinic for observation.
The crew were laying down the rods, employee put his hand on the loop of the pulling plug and went to slide the tube down on the truck, at the time there was slack in the line when the tube was all the way down on the bed the slack pulled out of the cable causing the clevis to move back and pinch his finger between the clevis and the loop of the pulling plug.
Driver had spotted at the shovel and set the wheel brake. As she was placing her hands on the steering wheel, the dipper released the first load, jarring the truck, causing her hand to slip through the steering wheel. The steering wheel spoke struck her right hand.
Employee was operating a rubber tire dozer when he passed through a rough area and stated the seat bottomed out, causing him lower back pain. The employee was taken in for examination, given prescriptions for pain and released after one day off work.

Employee was placing pipe in fusing machine to get ready to cut. The valve that clamps the cylinder on pipe clamped prematurely causing his hand to move the other valve which pinched his right hand.
While using Radial Drill Press, coolant splashed in employee's left eye when he leaned over to check center of hole.

Operator of the 523 HT was being loaded on the right side of the 11 SH. The shovel swung over the truck and a rock rolled off the teeth and struck the bed of the truck. The operator was jarred and the pain was instant in her neck and right side of her back.

While using the one inch impact wrench to remove plow bolts from the cutting edge of a D-11, some type of foreign material entered or fell into employee's left eye.
Employee was doing the monthly inspection on the Metcalf IOS Crusher 100 ton Overhead Crane. They were inspecting the Trolly Drum, Gear Boxes and Cables. They were in the process of moving the hoist drum when the rotating machinery caught his lanyard on his Safety Harness.
Employee smashed tip of middle finger under hydraulic press plate on strapping unit of # 4 machine strapping unit at Stargo Ew Tankhouse.
Employee was driving a haul truck. While stopped to load, the shovel bucket opened too soon causing the load to drop on the tail of the truck. This caused the bed to raise and fall heavily, jarring the driver. The driver sustained soreness in her back and neck.
Employee was operating a 793 Caterpillar Haul truck and was being loaded at a shovel. The shovel loaded the truck with some large material jarring the truck operator.

A locking impact universal impact socket came off the 1/2' impact while tightening the alternator bracket on 510911 and struck employee in the front teeth. The tooth was broken off almost at the gum line. Prescription meds were issued.
Employee was using a torch to remove a bolt on a ball mill in the west repair bay. He was holding the torch in his right hand and bracing the bolt with his left. The torch got too close to the bolt being cut and employee rolled torch away from bolt, and the flame ran down his left hand, burning the top inside of his left wrist.
While pushing the bit basket over the rail of the drill, the operator felt his right wrist pop.
Employee was using a jackhammer for concrete demo. The jackhammer hit a piece of rebar in the concrete causing the hammer to bounce out of his hands and land on his right shin.
Employee was walking back to the towers along the power cable when a coworker swung the basket to pull the slack in the cable to the back of the shovel. When the basket swung, the cable lifted up and struck the employee knocking him to the ground. This resulted in a fracture to the left wrist.
EMPLOYEE WAS LOADING A SCRAPER IN A BARROW PIT, HE CALLED THE D8 REQUESTING A PUSH. HE STOPPED DUE TO THE HARD SPOTS & THE DOZER HIT HIM, CAUSING HIS HARD HAT TO KISS THE STEERING WHEEL. HE WAS TAKEN IN TO THE E.R. & GIVEN TWO DAYS OFF WORK.
The operator of the 15 shovel reached down to adjust his seat in the cab when he had a sharp pain in his lower back.

EE WAS ARK GOUGIN, THE CARBBON ROD STUCK AND HOT PIECE FELL INTO THIS R BOOT COVER CAUSING A SMALL BURN. 1ST AID WAS ADMINISTRERED TO BURN, AND EE COMPLETED HIS SHIFT. ON 2/17 HIS LEG WAS HURTING AND TAKEN TO CLINIC AND WAS DETERMINED THAT THE LEG WAS INFECTED.
While using a torch to cut bolts, slag blew back on employee's wrist and forearm.
Employee was using grinder and rested the grinder on his leg before it stopped spinning.
