Tech was climbing up back of 637 Scraper between transmission and rear fender to change a pump. Tech had grease on bottom of foot and he slipped and fell backwards. As he turned to catch himself, he fell to the ground approx 4 feet. He fractured his left wrist trying to catch himself.

Employee complained of muscle spasm in his back while operating a 637 Cat scraper in tandem with another operator. Employee was transported to the ER where he was checked out and taken off work. Ongoing.
Employee drove his 775E haul truck over rough ground at the dump location and strained his arm as he held onto the steering wheel.
The employee was standing on a rock about 12" off of the ground loading secondary rounds in a larger rock behind it when his boot slipped off of the rock twisting his left knee.

Employee was taken into the ER room and given a tetnus shot and anit-biotics. He was refered to a specialist and scheduled to have surgery to repair the tendon on his left index finger (3/9/07) On Going
Employee was traveling to the SX on the access road when an oncoming contractor vehicle swerved to miss some deer on the road colliding head on with the employee. Employee was transported to the hospital via ambulance. Employee suffered from whiplash and contusions to her chest area. She was given pain medication and taken off work for two days.
Employee noticed that a valve on cell 20 was not close all the way. He went back to re close the inlet valve on cell G 20 when he splashed electrolyte that was on the floor and electrolyte got on his clothing and saturated them and electrolyte got on his right calf on his right leg.
Employee was climbing down from the bed of the CC-152 boom truck when he started descending the boom truck his right foot slipped off the rung, causing his right leg to hang up between the rungs on the dismounting ladder.

He flushed his eyes, but by Wednesday, 2-21, his eye was irritated. He was taken to the doctor, a small speck of metal was removed (not embedded), given antibiotics and returned to work.
The employee was attempting to clear a ball jam in the chute for the #4 mill. He put his hand in to the chute to move a loose ball and the following balls rolled forward trapping his hand in the chute. While trying to dislodge his hand with his other arm, he scrapped his forearm.
EE was installing conductors into conduit. He was pulling in a downward motion from above his head to below his waist pushing conductors into the conduit. This motion exerts force int he elbows and shoulders. The strain/sprain is in both elbows where the forearm and tricep ligaments attach. Employee states over a couple of months he has experienced pain in shoulder and arm as a result of lifting.
Employee was tightening bolts on a valve when the wrench slipped off bolt and struck employee above the right eye.

Employee removed the choker from the crane lowering the impactor at the crusher. The impactor shifted causing it to fall and caught his ankle between it and the old headnut on the floor. This resulted in a contusion to his left ankle.

Employee was pulling electrical wire up on cable tray on conveyor. The rope they were using to pull with came undone. The cable fell down and struck him on his hand causing a hairline fracture to his right index finger.
Employee was attempting to adjust the forks on the forklift when his hand became caught between the mast and the fork causing a contusion to his right hand.
Employee was removing used ripper points from the back of his service truck. After tossing the next to last ripper point and reaching for the last ripper point, the employee felt something impact his left tricep. He did not see anything and continued to reach for the last ripper point when he noticed blood trickling down his arm.

Employee was loading pulley onto truck using forklift when pulley rolled off of forks and smashed employee's fingers between bearing housing and torch box.

During shift change Employee was moving toward the back of the 140-0 van to be seated. The individual driving the van began to move while employee was still upright causing them to fall. The driver of the van didnt stop to make sure employee was in the seat.
Employee was driving water truck w/air seat, truck went over bump and employee's hard hat hit roof of cab causing him to bite down hard and fracture tooth
Drill string hung up and then dropped, smashing employee's right thumb between water swivel and chuck jaws.
