EE WAS WELDING. HE STOPPED AND LIFTED HIS HEAD. A PIECE OF SLAG POPPED AND HIT HIS NOSE AND EYE.
EMPLOYEE WAS CUTTING STEEL FOR PLANT REPAIRS.
EE WAS REMOVING A CUTTING EDGE BOLT, THE BOLT BROKE, FLEW UP AND HIT HIS HEAD. HIS HEAD WAS CUT REQUIRING 5 STITCHES TO CLOSE.
